高血壓是常見的慢性疾病,長期罹患高血壓容易造成心臟、腎臟及腦部的併發症,對於高血壓患者而言,如何防治並讓血壓維持恆定是一項重要的課題。
本研究的目的,在探討經鄭子三十七式太極拳運動對高血壓病患其血壓、體脂質、腰圍、焦慮程度及心臟自律神經活動狀態等之改善情形。研究以立意取樣法及準實驗方法,從太極拳協會團體中參與之34 位高血壓患者初次太極拳訓練者為實驗組,及某社區37 位高血壓患者為對照組進行研究,研究時間於2008年4月至7月。實驗組在太極拳老師的指導下進行太極拳運動訓練,運動頻率為每週3次,持續12 週;對照組則維持原有的生活型態。資料總計收集三次,包括前測、後測(訓練第六週) 及後後測(訓練第十二週) 資料。經統計分析結果顯示,兩組受測者在宗教信仰、年齡及每週運動次數有顯著差異外,其他人口學學變項如性別、規律運動運動強度及人格特質屬性皆無顯著差異,研究結果發現實驗組在六週之後,收縮壓顯著的降低。另一方面,透過F檢定,我們可以看出實驗組的後後測與後測的差距,顯著的有別於對照組的後後測與後測的差距;舒張壓實驗組後後測與後測的差異,跟對照組後後測與後測的差異,達到統計上的顯著差別;實驗組在體脂值實驗組後測與前測的差異及後後測與後測,皆達到統計上的顯著差別;腰圍部份實驗組後測與前測的差異及後後測與後測,也達到統計上的顯著差別;實驗組在焦慮程度後測與前測的差異及後後測與後測,皆達到統計上的顯著差別;血氧濃度測量顯示實驗組後後測與後測的差異,跟對照組後後測與後測的差異,達到統計上的顯著差別;與自律神經活性相關的變項,皆未達到統計上的顯著,研究結果顯示研究結果顯示經過6週太極拳運動訓練後使得高血壓患者之收縮壓、身體質量指數、腰圍、血氧濃度及焦慮程度獲得改善,且經過12週太極拳運動訓練後使得高血壓患者之收縮壓、舒張壓、體脂值、腰圍、血氧濃度皆獲得改善。
Background and purpose:Despite advances in medicine and clinical care, heart disease continues to be the leading cause of mortality in Taiwan. There is concern that the prevalence of heart disease will continue to rise because of increased affluence and adoption of a westernized lifestyle, cholesterol intake, stress, and a sedentary lifestyle among Taiwanese people. Tai Chi Chung, a widely accepted exercise in Chinese society, performs evenly from beginning to end with the same continuous rhythm in each posture, which will improve circulation and breathing the internal organs. Using Zen Tai Chi Chung 37 movements as an exercise mode, this study explored a 12-week exercise program would affect the health fitness conditions of hypertension patients.
Methods:seventy-one (71) community-dwelling hypertension patients were randomly assigned to experimental group (TCC; n = 34;Mean age = 58.1 years, SD = 8.8) or control group (CG; n = 37; mean age = 65.9 years, SD = 13.2). The experimental group met for 45 minutes, three days per week for 12 weeks. Outcomes measured before and after training included (two measuring points:6-week after training and 12-week after training) estimated blood pressure (systolic and diastolic blood pressure), body mass index (BMI), waist circumference, SPO2, analysis of HRV readings, compare them to the standard figures developed by European Society of Cardiology and the North American Society of Pacing and Electrophysiology in 1996, as a measure of autonomic control of the heart, and the anxiety status by Beck Anxiety Inventory (BAI) questionnaires.
Result:Significant improvement was seen on the blood pressure (systolic blood pressure averagely decreased 12.8 mmHg, diastolic blood pressure averagely decreased 6.6 mmHg) in the TCC group. Besides, thirty-four subjects from TCC demonstrated significant reductions in BAI scores from 5.0±5.68 to 0.90±1.91 at 12-week follow-up. As to the analysis result of HRV and compare of the normal values, there is no significant improvement.
Conclusion:TCC was found to be an effective way to improve blood pressure and anxiety status measures in elderly hypertension patients over a 12-week period exercise. TCC was also found no significantly enhancing to autonomic nervous modulation comparing to the normal values.
