本研究目的在了解幼兒園餐點營養供應之現況,並藉由營養教育介入方式,探討幼兒園餐點設計者營養知識、態度及餐點設計行為之改變狀況。第一部份:研究對象為台中縣55所幼兒園,以稱重紀錄方式評估幼兒園早點、午餐及午點之供應狀況。結果顯示,以衛生署幼兒膳食營養素參考攝取量(Dietary Reference Intake, DRIs)的二分之一做建議量時,總熱量達建議之95%;蛋白質、磷、鈉、鎂、維生素A、C、B1及B6在建議之上;而醣類、膳食纖維、鈣及維生素E則未達建議。其中公立幼兒園在蛋白質、脂肪、鈣、磷、鐵、維生素E、B1、B2、B6及菸鹼素之供應量皆顯著高於私立(p<0.05)。以幼兒每日飲食建議份數的二分之一作為六大類食物份數之建議量,奶類及水果類僅達建議之29%、47%,而蛋豆魚肉類供應量則超過建議量的2.3倍以上,其中公立幼兒園在奶類、水果類及蛋豆魚肉類之供應份數皆顯著高於私立(p<0.05)。以健康飲食指數(The Healthy Eating Index; HEI)來評估台中縣幼兒園餐點品質,其總分平均為64.9分,屬於「有待改善」範圍,其中在蔬菜類、水果類、奶類及鈉供應得分最低。私立幼兒園在水果類及奶類得分更是顯著低於公立(p<0.05)。
第二部份:將第一部份的55所幼兒園中,有參加兩週共八小時的營養教育課程者分為實驗組(N=26),而無參加者則分為控制組(N=33)。分別於營養教育介入前、後對二組進行營養知識、態度問卷調查及幼兒園餐點之供應評估。結果發現,介入後控制組之營養知識、態度總得分與教育前比較並無顯著改變,而實驗組之營養知識、態度總得分則較教育前有顯著提昇(p<0.05)。組間比較上,實驗組營養知識總得分顯著高於控制組(p<0.05),但在態度總得分組間並無顯差異。餐點設計行為部份,營養教育介入後控制組之各餐點設計行為與教育前比較並無顯著改變,而實驗組蛋白質供應量有顯著減少且膳食纖維、鈣供應量顯著增加(p<0.05),蛋白質減少及膳食纖維增加的幅度在組間有達到顯著差異(p<0.05);蛋豆魚肉類份數減少及水果類份數增加的幅度在組間亦有達顯著差異(p<0.05);實驗組只有HEI水果類得分比控制組有顯著的提昇(p<0.05)。本研究發現公立幼兒園、園所規模小及專業人員之餐點設計者營養知識較佳;大學畢業者營養態度較佳;而工作年數高者餐點設計行為較差。餐點設計者營養知識、態度及餐點設計行為三者之關係,營養知識與餐點設計行為皆與營養態度呈正相關(p<0.05),但是營養知識與餐點設計行為卻無顯著相關。
由上述結果可知,幼兒園餐點營養現況是不甚理想,尤其在醣類、膳食纖維、維生素E及鈣偏低,蛋白質、磷及鈉過剩的情況是目前幼兒園餐點應改進的問題。營養教育的介入對於餐點設計者營養知識、態度及餐點設計行為有明顯的增進,具有正面效果。因此,對餐點設計者持續實施營養教育以改善幼兒園營養狀況確實是必要的。
The purpose of this study was to investigate the nutritional status of meal supply and the effects of nutrition education intervention on nutrition knowledge, attitude and menu plan behaviors of menu planners in preschool daycare centers. First stage we collected meal data (lunch and two snacks) in fifty-five preschool daycare centers in Taichung County. We compared the data with one-half of the Dietary Reference Intakes (1/2DRIs). Data indicated that the average supply of total energy was achieved 95% 1/2DRIs. Supply of the protein, sodium, phosphorous, magnesium, vitamin E, B1, B2 and B6 were over 1/2DRIs. However, supply of carbohydrate, dietary fiber, calcium and vitamin E were below 1/2DRIs. In private daycare centers, the supply of protein, fat, calcium, phosphorous, iron, vitamin E, B1, B2, B6 and niacin were higher than public (p&lt;0.05). Then we compared the six major food groups data with one-half of the Food Guide for preschooler (1/2Food Guide). Data indicated that dairy and fruit groups were lower than 29 % and 47% 1/2Food Guide, respectively. Meat group was 1.2 times of 1/2Food Guide. In addition, the servings of meat, dairy and fruit groups in private daycare centers were higher than public (p&lt;0.05).The meal quality of daycare centers was assessed using the Healthy Eating Index (HEI). Most meals were evaluated that ''need improvement'' with a mean total HEI score of 64.9, with the lowest score for sodium, dairy products, vegetables and fruit groups. In private daycare centers, the HEI score of fruit and dairy products were lower than public (p&lt;0.05).
Second stage, fifty-five menu planners in preschool daycare centers were separated into experimental (E group, N=25) and control (C group, N=30) groups. E group was attended nutrition education program for two weeks (total 8 hours). C group was not attended any nutrition education program. Nutrition knowledge attitude questionnaires and meal data were collected before and after nutrition education. The results showed that the total nutrition knowledge and attitudes score of E group in post-test were significantly higher than pre-test (p&lt;0.05), the total knowledge score of E group were significantly higher than C group (p&lt;0.05). But there were no significant difference in the total nutrition attitudes score between E and C group in post-test. Regarding nutritional status of meal supply, no change was observed for C group in post-test. However, E group had decreased supply of protein, and increased supply of dietary fiber and calcium after intervention. Compared with C group, protein and dietary fiber have significant change in E group (p&lt;0.05). Regarding six major food groups, the servings of meat and fruit groups in E group were significant improvement in post-test (p&lt;0.05). Besides, the HEI score of fruit group in E group was higher than C group after the education (p&lt;0.05). The result demonstrated that menu planners of public centers, smaller-scaled centers or nurse and dietician professional had better performance in nutrition knowledge. Menu planners who graduated from university had higher score in nutrition attitude. The years of planning experience was negatively correlated with menu plan behaviors. There was a positive correlation among nutrition knowledge and attitude, attitude and behaviors (p&lt;0.05). However, there was no correlation between knowledge and behaviors.
To sum up, meal supply of preschool daycare center need to be improved. Carbohydrate, dietary fiber, calcium and vitamin E, protein, sodium and phosphorous supply insufficiency were problems of preschool daycare center in Taichung County. Furthermore, the nutrition education program significantly improved menu planner''s nutrition knowledge, attitude and menu plan behaviors. Therefore, nutrition education is necessary for menu planners in child care centers.
