臨床上,病人斷裂的周邊神經接受顯微手術接合的時間愈早愈好,然而延遲縫合卻在臨床上很常見。我們先前的研究顯示,受損顏面神經的立即縫合可避免神經斷裂所引起的病理變化,包括中樞顏面神經內強烈的膠細胞反應及顏面運動神經元細胞內自由基相關酵素表現的劇烈改變。然而,延遲縫合的影響則未知。我們以雄性SD大鼠為實驗動物,將其左側顏面神經截斷後三小時再縫合。受傷顏面神經的再生情形以下列幾種方式評估,包括觸鬚擺動及眼瞼閉合的觀察、神經傳導檢查、神經肌肉接合的分析,以及逆向追蹤術。我們發現延遲縫合後,受損神經功能恢復與再生的時程及程度,與立即縫合的結果相似。但是延遲縫合引起中樞顏面神經內輕微的微小膠細胞及星形膠細胞反應,立即縫合後並不會看到這樣的情況。除此之外,手術後一至十週,延遲縫合造成顏面運動神經元細胞內氧化壓力傷害指標產物3-nitrotyrosine及4-HNE的表現量小幅度但有意義地增加,且造成nNOS、iNOS、calcineurin、catalase、Mn-SOD及Cu/Zn-SOD的表現量出現小幅度變化。這些實驗結果顯示,與立即縫合相較,延遲三小時縫合並不會明顯妨礙損傷顏面運動神經元的功能恢復及軸突再生,但會造成中樞顏面神經內的輕微膠細胞反應及神經元細胞內氧化壓力的小幅度增加。
Clinically, the microsurgical repair of interrupted peripheral nerve is required to be as soon as possible, however, delayed nerve sutures are common events. In our previous study, we found that immediate facial nerve repair prevented axotomy-induced drastic glial reaction in central facial nerve and dramatic changes of expressions of free radical-related enzymes in facial motoneurons (FMNs). However, the effect of delayed facial nerve repair remains unknown. Male SD rats received left facial nerve resection and then 3 hours-delayed suture. The regeneration of injured facial nerve was measured by observation of vibrissae whisking and eye closure, nerve conduction study (NCS), analysis of neuromuscular junction (NMJ), and retrograde tracing. We found that the time course and the extent of functional recovery and nerve regeneration after delayed suture were similar to immediate suture. Nevertheless, delayed suture caused mild microglial and astrocytic reactions in the central facial nerve, which were not observed after immediate suture. In addition, delayed suture induced a slight but significant increase of markers of oxidative damage (3-nitrotyrosine and 4-HNE) and a small fluctuation of nNOS, iNOS, calcineurin, catalase, Mn-SOD, and Cu/Zn-SOD expressions in FMNs during 1-10 weeks after nerve repair. These results suggest that compared to the immediate nerve repair, a 3 hours-delayed suture does not apparently impair the functional recovery and axonal regeneration of injured FMNs but significantly results in mild glial reaction in central facial nerve and a slight increase of oxidative stress in FMNs.
