國內兒童緩和醫療仍在萌芽階段,但兒童加護病房的父母面臨孩子生命末期的醫療決策卻日趨頻繁。在加護單位的醫護人員著重於提供穩定孩子生理的各種照護,選擇不施行心肺復甦術勢必帶給父母不少的衝擊。在父母簽署不施行心肺復甦術同意書的過程中,如果無法滿足父母的需要時,勢必影響父母日後的身心健康。因此本研究將針對生命末期孩子的父母,探討他們簽署DNR同意書的經驗。
本研究採用質性研究中之深度訪談法,研究對象採立意取樣輔助滾雪球的方式(snowball sampling),係以臺北市某醫學中心曾經簽署不施行心肺復甦術孩子的父母,資料收集期間自2008年9月至2009年1月止,共訪18位父母。本研究之訪談資料以內容分析法歸納結果分成父母簽署不施行心肺復甦術同意書的感受、自我調適、及影響因素三個層面來說明及討論其主題。本研究透過對父母為生命末期孩子簽署不施行心肺復甦同意書經驗之了解,研究結果可提供臨床護理人員在照護生命末期孩子及父母之參考,進而提升其照護品質。
The development of palliative care for children in Taiwan is at the beginning stage, but the chances of the parents to make medical decisions for their children at end of life increase. The medical staff in pediatric intensive care unit usually focuses on providing care to stabilize the physiologically compromised child. Choosing not to resuscitate will have a huge impact on the parents and may affect their wellbeing in the future. The purpose of this study is to understand the parental experience of signing Do Not Resuscitate form for their child at end of life.
Data were collected in a medical center in Taipei, between September, 2008 and January, 2009. Purposive and snow-ball sampling were used. In-depth interviews were conducted with 18 parents who had signed the form. The data were analyzed using qualitative content analysis.
The results were reported and discussed based on three perspectives, parent’s feeling about signing DNR, parent’s adjustment about signing DNR, and the influencing factors toward signing DNR. The results can provide the medical staff as a reference for their clinical care.
