教師由於工作的需求,必須長時間用聲,為嗓音異常的高危險群。雖然國內外已有研究探討教師的嗓音問題,但是此類研究多以問卷調查嗓音自覺情況,鮮少與聲學分析的結果比較,無法解釋生理上的改變。本研究目的是探討自覺嗓音正常與異常的女性教師嗓音在聲學分析上的差異,分析聲學參數與自覺嗓音上的關係,以及找出預測自覺嗓音異常之聲學指標。
本研究使用聲學儀器,分析80名女性教師嗓音樣本。其中,40 名自覺嗓音異常(VD),40 名自覺嗓音正常(NVD)。其中包括閱讀短文的基頻與音強,以及發長母音/阿/之頻率變動率、振幅變動率、與噪音訊號比。使用T檢定、邏輯回歸、以及圖示法之探索性資料分析 (Exploratory Data Analysis, EDA) ,比較各參數在兩組的差異、嗓音自覺情形與聲學分析的關係、以及篩選自覺嗓音異常的指標。
結果發現,NVD及VD在說話基頻與說話音強均無顯著差異。但VD的頻率變動率、振幅變動率、與噪音訊號比皆顯著高於NVD。此外,基頻和音強參數與自覺嗓音問題沒有顯著關係,但頻率變動率、振幅變動率、與噪音訊號比均與自覺嗓音狀況有顯著關係。其中,Noise to Harmonic ratio和Shimmer in dB最能反應嗓音自覺狀況。若將Noise to Harmonic ratio>=0.138且Shimmer in dB>= 0.470 dB當作預測指標,可以篩選出60%目前無嗓音異常,但未來會出現自覺嗓音異常的教師。
本研究發現自覺嗓音異常之聲學檢測方法,以及預測嗓音異常之聲學指標。可提供臨床評估、治療、及預防嗓音異常之依據。
Teachers are most likely to develop voice disorders because they have to speak a lot for occupational needs. Although there have been many studies on teachers’ voice problems, most of them are questionnaire studies. The pathophysiology and the relationship between acoustic data and self-reported symptoms were less mentioned. This study focused on the difference of acoustic parameters between the normal and abnormal self-reported teachers, the relationship between acoustic parameters and self-reported symptom, and the acoustic indexes to predict self-reported voice disorder.
Voice samples of 80 female teachers are analyzed with acoustic measurements. Forty teachers of the 80 are self-reported voice disorders (VD), and forty are self-reported normal voice (NVD). The acoustic parameters include frequency and intensity in reading, frequency perturbation, amplitude perturbation, and Noise-to-Harmonics ratio in sustained vowels. T-test, logistic regression, and Exploratory Data Analysis (EDA) was used to analyze the differences between the two groups, the relationship between self-reported voice status and acoustic parameters, and the index to predict self-reported voice disorders.
The results showed that no significant difference was found of fundamental frequency and intensity in reading between the VD and NVD. However, pitch perturbation, amplitude perturbation, and Noise-to-Harmonics ratio of VD were significantly higher than those of NVD. Besides, there was no significant relationship between the self-reported voice problems and the parameters of fundamental frequency and intensity. However, self-reported problem was significantly related with pitch perturbation, amplitude perturbation, and Noise-to-Harmonics ratio. Among them, Noise-to-Harmonics ratio and Shimmer in dB are the most important parameters for self – reported voice disorders. In addition, if Noise to Harmonic ratio >= 0.138 and Shimmer in dB >= 0.470 dB were set as predicting indexes, the successful rate to predict self-reported voice disorders for NVD is 60%.
This study investigates the acoustic parameters for self-reported voice disorders, and the acoustic indexes to predict voice disorders. These findings provide scientific evidence in clinical evaluation, treatment, and prevention of voice disordered patients.
