由於資訊科技及網際網路的蓬勃發展,醫療照護產業逐漸備受重視。病歷的演進從傳統的紙本病歷到病歷電子化,再轉變到電子化病歷階段,大大提升了醫療照護的效率及品質。雖然電子病歷帶來許多好處,但完整的照護資訊卻只存在醫院系統中,病患無法自行瀏覽及管理。近年來,伴隨著健康意識高漲,民眾對於自我身體狀況更加關心,因而產生個人化健康記錄的概念,民眾可以直接經由網際網路存取包含免疫,過敏症、病史、手術、及治療等資訊,也可記錄平日之體重及血壓等身體狀況供給醫生做參考,更加提升醫療照護的品質。
本研究採用健康資訊產業整合 (Integration the Healthcare Enterprise, IHE) 所提供之跨院病歷分享 (Cross-Enterprise Document Sharing, XDS) 規範文件中所定義之分享架構作為系統的基礎平台,並在文件註冊伺服器中建立本體論,希望能夠透過更有效率的語義查詢方式找出所需資訊。此本體論將整合用來描述文件的中介資料 (metadata) 、國際疾病分類臨床修訂第九版 (International Classification of Diseases, Ninth Revision, Clinical Modification, ICD-9-CM) 、以及都伯林核心集,讓病患可透過此本體論查詢出與自己有相類似病症病患所註記的網路資源,提供溝通管道以達到文件及知識共享的目的。

Owing to the rapid development in information technologies and computer network, the healthcare industry has become more important. The evolution from a paper-based medical record to an electronic-based one has promoted the healthcare with good efficiency and quality. Although the electronic health records provide a lot of benefits, the whole care information is just stored in the health information system. However, patients cannot review and maintain the records by themselves. Recently, the PHR concept has been brought up, in which people not only can access the information online about their own immunization, allergy, historical illness, surgery, and therapy, etc., but also can store their record the daily health data, such as body weight, and blood pressure, and so on, for physicians via Internet. It will more promote the healthcare with better quality.
This research will use the XDS profile offered by the IHE as the basic architecture to build a PHR system. Through setting up ontology in the document registry to provide the more efficiency way figures out needed information by semantic search. This ontology is combined document metadata, ICD-9-CM, and the Dublin Core Metadata Element Set for patient searching the registry to find useful Web resources which recorded by other patients with similar diagnostic. By this architecture, it can achieve the document and knowledge sharing objective.
