隨著年紀增加,下肢血液循環不良,導致阿基里斯腱老化,肌腱機械性質改變,進一步影響關節角度和步態,最後可能導致斷裂,臨床上雖可以超音波影像和磁振造影輕易診斷肌腱的部份斷裂與完全斷裂,卻無法判斷肌腱老化的狀況,故本研究嘗試以自然頻率來量化人體中阿基里斯腱的機械性質及老化所造成的變化。本實驗以年齡20歲左右年輕人(N=28)以及50歲以上年長者(N=21)為受測者,利用超音波動態影像,加速規與衝擊鎚結合,進行系統鑑別萃取自然頻率,量取自然頻率。超音波更可以量測深層軟組織自然頻率,實驗結果顯示老人族群的正規化自然頻率高於年輕族群,同族群的男女之間並無明顯差別,dosi-flexion角度越大造成頻率越高,並且肌肉強度不會影響肌腱的自然頻率,正規化自然頻率正比於肌腱軸向硬度,最後提出一個狀態指標-正規化自然頻率作為臨床診斷肌腱硬化或判斷老化的依據。
Blood supply and the number of capillaries per unit area in the calf decrease with age. Aging of the Achilles tendon leads to the alteration of mechanical properties of the Achilles tendon and influence ankles and gaits further and finally rupture the Achilles tendon. Partial rupture and complete rupture of tendons are easily diagnosed by ultrasonography and magnetic resonance image for clinical orthopedics. Clinically ultrasonography and MRI does not reveal the aging state of the Achilles tendon. The purpose of this study is that natural frequency is used to assess the change in mechanical properties of aging Achilles tendon in vivo. In this study, all subjects are divided into two groups, one is a normal age group at approximately twenty (N=28) and the other is the elderly group beyond fifty (N=21). A hammer is used to stimulate the tendon and measure vibration from B-mode dynamic images and an accelerometer. Signals of an impulse hammer and vibration signals from dynamic images and an accelerometer are used to extract natural frequencies. In the future natural frequencies of deeply soft tissues can be estimated by means of ultrasonography. The results show that normalized natural frequencies of Achilles tendon in the elderly group are higher than those in the young group. Gender does not affect natural frequencies; large angle of dorsiflexion results in high natural frequency, and the natural frequency is not related to muscle strength. Because normalized natural frequency is proportional to the axial stiffness, a dimensionless state index (normalized natural frequency) is suggested to reflect aging of Achilles tendon and to diagnose calcification of Achilles tendon clinically.
