研究背景與目的:隨著高齡者與身心障礙者的人口逐年攀升,其需藉由電動代步車維持日常生活功能獨立,以及協助其執行活動及參與的潛在行動功能受限之人口亦隨之增加。然而,目前在台灣關於電動代步車的需求評估多侷限於使用者的身體功能層面,鑒於身心障礙者權益保障法的落實,日後輔具的需求評估將以國際健康功能與身心障礙分類系統(International Classification of Functioning, Disability, and Health, ICF)為主軸,加入活動及參與以及環境與個人因素作為個人健康狀況與輔具產品需求的描述。基於內政部多功能輔具資源整合推廣中心已新擬出電動代步車需求評估準則,故本研究的目的為以國際健康功能與身心障礙分類系統調查台灣使用電動代步車的身心障礙者特性及其使用狀況,並同時驗證新擬之電動代步車需求評估準則之合適性。研究方法:本研究透過有意願合作的單位之協助,徵詢符合本研究收案條件之使用電動代步車的身心障礙者參與,並利用自行設計之半結構式問卷來調查電動代步車使用者的身體功能、活動及參與、環境與個人因素以及電動代步車使用前後差異與滿意度,作為驗證電動代步車需求評估準則的自變項。研究結果:本研究共收取電動代步車受訪者85人,受訪者男女性別比例約為6:4,其平均年齡為56.0(±14.6)歲,而居住地點包含台北縣市有24人(28.2%)、彰化縣有36人(42.4%)和嘉義縣市有25人(29.4%);在疾病診斷方面以腦中風有25人(29.4%)為多數;受訪者的身心障礙類別最多的是肢體障礙有74人(87.1%),障礙程度以重度有64人(75.3%)居多。本研究90%以上的受訪者在與電動代步車使用相關之身體功能為沒損傷,且其安全操控電動代步車技巧之能力與表現亦為沒困難,然其短距離與長距離步行之能力與表現多數為完全困難。本研究受訪者的心智功能之定位(定向)、注意力及記憶功能和感官功能與疼痛之視覺敏銳度及視野功能與神經肌肉骨骼與動作有關功能之手部功能,分別與多項電動代步車操控技巧有相關性,且在統計上達到顯著差異(p<0.05)。結論:電動代步車使用者多數為步行能力有困難者,且使用者的肢體、視覺及認知功能,與電動代步車的安全操控技巧有密切關聯,加上其主要生活領域的環境設置亦會對電動代步車的使用造成影響,故驗證電動代步車需求評估準則之合理性。本研究建立的電動代步車使用者特性之基礎資料,亦可作為未來相關研究的參考依據。
Background: As the population of elders and people with disabilities were increasing year by year, the latent population, with mobility limitations, using scooters for improving the independence of activities of daily living and social participations were also increased. The needs assessment for scooters in Taiwan mostly emphasized more on physical function of scooter users presently. However, after the Protection Act for Rights and Interests of (Physically and Mentally) Disabled Citizens were passed, the needs assessment of assistive technology will be in accordance with International Classification of Functioning, Disability, and Health, ICF, involving body function and structure, activity and participation, and environmental and personal factors for descriptions of health conditions and assistive technology needs. Furthermore, the center for assistive technology resources and popularization had drafted the verification of needs assessment criteria of scooter. Purpose: To investigate the characteristics of scooter users with disabilities in Taiwan based on ICF and verify the needs assessment criteria of scooter. Study design: An exploratory research design. Methods: We recruited scooter users who met the inclusion criteria through the assistance of departments with collaborative intentions, and investigated their body function, activity and participation, environmental and personal factors, differences between pre- and post-use of scooter, and satisfactions by a semi-structured interview questionnaire based on the ICF for verification of needs assessment criteria of scooter. Results: 85 subjects were recruited, and the ratio of male to female was 6:4. The average age of total subjects was 56.0(±14.6), and the residence locations included Taipei city and Taipei county, 28.2%, Changhua county, 42.4%, and Chiayi city and county, 29.4%. The majority of subjects’ medical diagnosis was stroke, 29.4%, category of disabilities was extremities disability, 87.1%, level of disabilities was severe, 75.3%. About 90% subjects had no impairment on scooter-related body functions, had no limitation with ability and performance in safe-control techniques of scooter, and had total limitation with ability and performance in walking short and long distances. As the results of Fisher’s exact test, subjects’ orientation, attention, and memory of mental functions, visual acuity and visual field of sensory functions and pain, and scooter-related hand functions of neuromusculoskeletal and movement-related functions had statistically significant differences with safe-control techniques of scooter respectively(p<0.05). Conclusions: Scooter users mostly have limitation in walking ability, and their extremities, visual, and cognitive functions are related with safe-control techniques of scooter, but otherwise environmental settings of major life areas still have influence on scooter use. Therefore, the rationality of needs assessment criteria of scooter has been verified, and the foundational data of characteristics of scooter users built by our research could be the reference resources for related future research.
