過去十年中,科學性論文及醫學相關文獻發表呈現爆炸性的蓬勃發展,健康專業人員在短時間內,以有系統的方法獲得可應用的知識益形重要。加上我國教育水準的不斷提升,護理人員面對愈來愈多能迅速及聰明的由各種管道取得醫療資訊的消費者,而經歷更大的挑戰。為回應病患與其家屬的問題及提供有效的幫助,護理人員應更致力於更新自己在尋求健康照護資訊的知識與能力。本研究之目的首在調查臨床護理人員在三類健康照護資訊的使用行為(包括非線上資源、醫學電子資料庫、以及網路使用);再以Davis等人所發展之科技接受模式為理論基礎,進一步探討臨床護理人員在於兩種資訊資源型態(醫學電子資料庫資源、網路資源)之影響其使用的因素。本研究採橫斷式的研究設計,以問卷調查法收集資料,針對台北市389位臨床護理人員調查,研究發現有:1.在三類資訊資源中,以醫學電子資料庫資源的使用頻率是較低的,約半年至一年使用一次。2.受試者傾向認同醫學電子資料庫資源、網路資源使用的認知有用性、使用態度、使用行為意圖的同意程度皆趨正向,除醫學電子資料庫資源使用之認知易用性的同意程度是趨於負向。3.在醫學電子資料庫資源的使用上,外部因子之資訊素養、職稱和認知有用性有關。外部因子之性別、教育程度、參加過醫學電子資料庫及網路資源教育訓練課程、資訊素養、家中有否電腦設備與認知易用性有關。4.在網路資源的使用上,外部因子之年齡、工作年資、進階職級、資訊素養和認知有用性有關。外部因子之教育程度、年齡、是否參加過醫學電子資料庫資源教育訓練課程、資訊素養、工作場所能否上網、以及家中有無電腦設備與醫學電子資料庫資源之認知易用性有關。5.多變項分析顯示在醫學電子資料庫資源的使用上,認知有用性的預測因子是認知易用性、資訊素養,共可解釋7.0﹪的總變異量。認知易用性的預測因子是資訊素養、教育程度、性別、參加過電子資料庫教育訓練課程,共可解釋8.7﹪的總變異量。使用態度的預測因子是認知有用性、認知易用性,共可解釋38.6﹪的總變異量。使用行為意圖的預測因子是使用態度、認知有用性,共可解釋46.8﹪的總變異量。使用行為的預測因子是資訊素養、使用行為意圖、認知有用性,共可解釋25.7﹪的總變異量。6.網路資源使用方面,認知有用性的預測因子是資訊素養、年齡、認知易用性,共可解釋6.3﹪的總變異量。認知易用性的預測因子是資訊素養、家中有否電腦設備,共可解釋13.7﹪的總變異量。使用態度的預測因子是認知有用性、認知易用性,共可解釋31.7﹪的總變異量。使用行為意圖的預測因子是使用態度、認知有用性,共可解釋58.8﹪的總變異量。使用行為的預測因子是資訊素養、使用行為意圖、認知有用性,共可解釋27.6﹪的總變異量。基於本研究發現而建議:以多元化管道方式提供護理人員更新專業知識仍是必要。未來應加強可提升護理人員認知易用性、認知有用性、以及資訊素養的策略,以加強護理人員的健康照護資訊使用行為的能力。
In the past decade, scientific papers and relevant documents publishing appeared
explosive flourishing development. To obtain modern knowledge using systematic methods within shortest time for health professionals becomes more important. Moreover with the increasing of education level in our country, nurses face more and more consumers who can obtain medical information quickly and smartly from diverse channels experience a great challenge. In order to response patients and their families’ problems and to offer effective helps, nurses must be more powerful for updating their knowledge and skills in searching healthcare information. In this study, we firstly investigated clinical nurses’ using behavior among three kinds of healthcare information (including “non- on-line resource use”, “electronic database use” and “internet use”). Then, using the Technology Acceptance Model (TAM) as the theory base which was developed by Davis et al, we explored the influencing factors of two kinds of information resources (electronic database and internet resources use) for nurses. A cross-sectional research design was conducted with a quota-sample of 389 clinical nurses worked in Taipei. Questionnaires were used to collect data. The main findings were concluded as following: (1) In three kinds of resources, the frequency of electronic database use was lowest, the internet use was the highest. (2) For electronic database use and internet resources use, subjects tended to agree with the values of usefulness (U); subjects also revealed a positive attitude (A) as well as an affirmative behavior intention (BI). However the perception of EOU of electronic database was toward negative. (3) In electronic database us, the external variables including information literacy, job position were related to usefulness (U). Gender, educational level, previous experiences in electronic database and internet training, information literacy, and computer access in households were related with ease of use (EOU). (4) As to internet resources use, age, seniority of job, promotion levels of work, information literacy were related with usefulness (U). Educational level, age, previous experiences in electronic database training, information literacy, computer access in workplace, and computer access at home were related with EOU. (5) Multivariate analyses on electronic database use, we found EOU could be as a predictor of U and accounted for 7.0% of the variance. Information literacy, educational education, gender, previous experiences in electronic database training program were the predictors of EOU accounting for 8.7﹪of the variance. U and EOU were the predictors of attitude (A) and accounted for 38.6% of the variance. A and U were the predictors of BI and accounted for 46.8% of the variance. Information literacy, BI, and U were the predictors of behavior (B) and accounted for 25.7% of the variance. (6) In internet resources use, information literacy, age and EOU were the predictors of U and accounted for 6.3% of the variance. Information literacy, and computer access at home were the predictors of EOU and accounted for 13.7% of the variance. U and EOU were the predictors of attitude (A) and accounted for 31.7% of the variance. A and U were the predictors of BI and accounted for 58.8% of the variance. Information literacy, BI, and U were the predictors of behavior (B) and accounted for 27.6% of the variance. Based on our findings, we suggest that diverse channels should be provided to nurses to update their professional knowledge. Strategies for increasing nurses’ ease of use, usefulness as well as information literacy should be stressed to empower nurses using behavior of healthcare information.
