繁忙的護理人員常需花費許多時間於處理文書記錄上,因手寫資料的重覆登錄常造成錯誤與不完整性,且相關醫療資訊未整合,更增加了在查詢資料與照顧上的困難度,尤其是在針對長期接受規則血液透析病患上,數十年累積資料常造成單位在病歷管理的困擾及護理照護品質降低。研究中以某醫學中心血液透析室為例,護理人員為改善既有資訊室(DOS)作業系統,利用「使用者自建」模式,自組系統開發團隊,以Excel VBA巨集進階技術建置輔助確立護理資訊系統需求,並在系統開發過程中,分析護理人員在資訊化需求、決策與執行過程中可能面臨的阻力、助力與挑戰的歷程,並探討護理人員自建資訊管理系統的可行性。
藉由護理資訊專家指導,同仁自組開發團隊親自參與、學習並建構資訊管理系統,過程中並詳實的記錄單位內在開發護理資訊管理系統之歷程。不但因此解決了同仁在照護血液透析病患時傳統文書作業上的不便與缺失,並加強相關醫學資料在收集時的便利性與完整性。研究方法為田野調查、實際記錄直接間接工作內容,收集護理人員對資訊化的需求,待系統建置完成後,利用TAM科技接受模型理論所設計的「血液透析護理系統雛型評估問卷」,調查單位使用過該系統的護理人員共計51位,以PLS(部分最小平方法)進行統計。結果顯示認知易用性對認知有用性有較強烈的顯著影響,而認知易用性會直接且間接的影響認知有用性,且對使用態度與使用行為意向均有顯著影響(P<0.001),為本雛型護理資訊系統推行初期藉以預測使用者行為的最關鍵因素。
研究中並建議:單位以自組系統開發團隊來自建護理資訊管理系統的合作學習模式,相當值得推廣。管理者未來將可利用所得資料加以彙整為資訊,並與帳務、病人安全、人力資源等相關管理作業系統資源整合、分析、研究,進而運用在臨床照護工作與顧客關係管理上,將能有效簡化護理人員在執行病歷記錄的時間,提升對長期血液透析病患的服務與照護品質。
The busy nursing staffs need to spend many time frequently to process in the copy clerk record, because hand-written materials redundant registers creates the mistake and the imperfection frequently. Moreover, the related medical information conformity had not increased in the inquiry material and the attendance degree of difficulty, especially aims at accepts in for a long time the regular hemodialysis sickness, dozens of year accumulation material creates unit''s looks after the quality in medical record management puzzle and nursing frequently reduction. In the research take some medicine center hemodialysis room as the example, the nursing staffs to improve already has the information room (DOS) the work system, the use “the user from constructs” the pattern, forms the system development team voluntarily, great collection enters the step technology by Excel the VBA to establish the auxiliary establishment nursing information system''s demand, and in the system development''s process, the analysis nursing staffs the resistance which, the boost and the challenge course in the information demand, policy-making and in the execution process possibly faces, and discusses the nursing staffs to establish the information management system management system''s feasibility voluntarily.
Because of the nursing information expert''s instruction, the colleague develops the team from the group, and participates, studies and constructs the information management system management system personally, in process and course of the detailed record unit intrinsic development nursing information management system management system. Not only has therefore solved the colleague when looks after the hemodialysis sickness in traditional copy clerk work inconveniencing with the flaw, and strengthens the related medicine material in collection time convenience and the integrity. The research technique was the field investigation, the real record direct and the indirect work content, the collection nursing staffs to the information demand, after the waiting system establishes completed, using the TAM science and technology accepts “the hemodialysis nursing system embryonic form appraisal questionnaire which” the model theory designed, the investigation unit has used this system''s nursing staffs total 51, (partial least squares) carried on the statistics by PLS. The result showed that the cognition usability has the intense remarkable influence to the cognition usefulness, but the cognition usability direct and indirect influence cognition usefulness(P<0.001), Forecasts the user behavior so as to for this embryonic form''s nursing information system carrying out initial period the most key aspect.
In the research suggested: The unit by from constructs the nursing information management system management system''s cooperation study pattern from the group system development team, is quite worth promoting. The superintendent might use the obtained material in the future to collect entire is an information, and with bookkeeping, patient safety, human resources and so on related management work system resources conformity, analysis, research, then utilization, in clinical looks after works relates in the management with the customer, will be able to simplify the nursing staffs effectively in the execution medical record time, promotes with to look after the quality to the long-term hemodialysis sickness service.
