隨著少子化、健保服務普及、GDP持續增加，國人之平均壽命明顯地逐年延長，致人口結構快速老化，國民醫療保健支出顯著增長。2009年台灣總生育率，再次往下跌至1.02人，創下全球最低的出生率，成為全世界少子化最嚴重的國家。2008年達65歲以上老年人口比例，又已超過10%以上。台灣的經濟和人口結構如此急遽改變，將如同多數先進國家一樣，承受著醫療保健需求持續增長的壓力，故人口老化及醫療保健支出情況是值得深入探討的課題。
本研究檢視我國經濟成長(GDP)、人口老化(AP)和醫療保健支出( NHE)之長期關係為實證對象，樣本資料取材自衛生署2008年國民醫療保健支出統計，樣本觀察期間共計18年。研究步驟先使用ADF、PP、NP 單根檢定，再採用Johansen共整合檢定進行分析，探討三變數間是否具有長期均衡關係。實證結果發現三變數間具有單根，且正向發展的長期共移均衡關係。長期觀之，我國醫療保健支出將隨經濟成長及65歲以上人口增加而增長，三變數間具有長期共同可推測的趨勢。
本研究結果提供政策制定者，經由經濟成長率及人口結構變化之推估，正確決定醫療保健支出之年增率。經濟成長率是總體經濟指標，而人口結構變化影響國力強弱與國勢消長至鉅。以此兩項重要資訊為依據，估算未來醫療保健支出，方能適度控制該項經費額度，不致造成國家財政上的超額負擔。

The population structure in Taiwan has rapidly changed due to a substantial decline in birth rates, the extension of health care services, the continuous GDP growth in the past two decades, and the increasing average life expectancy. Consequently, national expenditures on medical care have increased significantly. With a fertility rate of 1.02, Taiwan was ranked number one among countries of low fertility rate worldwide in 2009. The long-term policy and expenditure of medical care services have therefore become increasingly important.
We took the Gross Development Per Capita (GDP) and aging population (AP) data with the national health expenditure (NHE) information published by the Department of Health, Executive Yuan, R.O.C.(TAIWAN)(2008) as three variables to analyze the relationship among them from 1991 to 2008 by using time-series analysis ,ADF、PP、NP Unit root test and Johansen co-integration test. The results suggest a positive unit-root and a long-term co-integrated equilibrium relation among GDP, AP and NHE. NHE in Taiwan is expected to increase in the long-run when the economic growth and the number of residents aged 65 or over continue to grow.
The results of this study will provide policy-makers the path to accurately estimate the annual growth rate of medical care expenditures through tracking the economic growth rate (EGR) and the change of population structures (PS). Economic growth rates and population structures are two strong indicators for the trend of the aggregate economic development and the political power of a nation. By using these two variables in forecasting, policy-makers can accurately predict aggregate expenditures and hence have the budget deficits well controlled.
