本研究旨在探討治療式遊戲介入，對癌症兒童及青少年體外放射線治療期間的焦慮心理及生理表現之影響，以及與主要照顧者間焦慮相互影響之關係。研究設計採類實驗設計，於北部某醫學中心之癌病中心放射腫瘤科，針對第一次接受體外放射線治療之3-18歲癌症兒童及青少年進行收案，介入組以治療式遊戲介入收案共9名，對照組收案共10名。焦慮心理反應探討部分，病童以貝克兒童及青少年焦慮分量表 (The Beck Youth Anxiety Inventory，BAI-Y )、臉譜量表 (Facial Image Scale , FIS ) 等心理指標為主，主要照顧者則使用成人貝克焦慮量表中文版 (Beck Anxiety Inventory，BAI)；心率變異率 (Heart rate variability , HRV)、生命徵象 (Vital Sign) 以及唾液皮質醇 (Salivary Cortisol) 等作為焦慮生理反應的指標。測得的數據皆以平均值 ± 標準差 (Mean ± Standard Deviation) 表示，以獨立樣本t 檢定 (Independent t test) 分析組間各項變數的差異，以相依樣本t檢定 (Dependent t test) 分析組內個案變項前後測間的差異，以單因子變異數分析 (One Way Anova) 分析治療式遊戲介入對於焦慮反應的影響，各項分析均以P <0.05視為顯著水準。結果顯示，兩組病童的焦慮心理及生理反應表現在治療式遊戲介入前並無差異；治療式遊戲介入後，體外放射線治療前，介入組在臉譜量表、心率變異率的高低頻功率比及唾液皮質醇的變化達統計上之顯著，顯示治療式遊戲的介入，確實有效降低進行體外放射線治療前的病童焦慮程度。同時，介入組主要照顧者的前後測心率變異率的高頻功率及正規化高頻呈現顯著差異，顯示介入組的焦慮程度低於對照組。病童與主要照顧者，在焦慮生理反應部分，如心率變異率、生命徵象及唾液皮質醇濃度及含量的變化呈現正相關性。研究工具的相關性分析顯示，病童的臉譜量表與心率變異率的低頻功率及心率變異率、唾液皮質醇含量與心率具有統計上之顯著相關，顯示心理量表無法精確掌握立即的焦慮所引起的焦慮反應；主要照顧者方面也顯示成人貝克焦慮量表、心率變異率及唾液皮質醇濃度之間，在測量焦慮程度上顯著相關。治療式遊戲的介入除提高癌症治療品質，減少醫療成本外，更使接受體外放射線治療之病童，得以無懼完成治療，進而降低對病童身心可能潛在的負面影響。

The purpose of this study was to investigate the effect of therapeutic play on the mental anxiety and physical reaction of cancer children and youngsters treated by external beam radiotherapy. The interaction of mental anxiety between the major attendance and the patient, and the correlation between measurement tools were also testd. We measured the Beck youth anxiety inventory (BAI-Y), facial image scale (FIS) for the patient. The Beck anxiety Inventory (BAI) was performed for attendance. Physical anxiety is based on the measurement of heart rate variability (HRV), vital sign, and salivary cortisol, and measured for all the participants. The results suggested that children in the experimental group reported significantly lower anxiety scores and exhibited fewer negative emotions before external beam radiotherapy than the children in the control group. The anxiety between the patients and the attendances is interactive significantly. There are significant correlation between FIS and low frequency power, FIS and HRV, salivary cortisol and heart rate in patients. The significant correlation also presented between BAI, HRV, and salivary cortisol. Therapeutic play not only improves the medical quality and reduces the unnecessary medical human resources and expense. It also induces children to courageously receive and finish radiotherapy. As a result, it further decreases the potential negative influence on the children patients.
