內皮細胞在血管中是扮演一個不可或缺的角色,它能調節血管張力、維持血壓穩定。當血管中的內皮細胞受損時會導致內皮細胞功能異常,進而增加動脈粥狀硬化的形成,引發心血管疾病的產生(高血壓、心臟病、糖尿病及中風等)。許多研究證實內皮細胞功能異常與動脈硬化及心血管危險因子皆具有相關性。因此評估血管中的內皮細胞功能可以早期偵測動脈粥樣硬化的形成,達到早期發現早期治療的效果。
  本論文研究使用了兩種不同的演算法來評估健康族群與第二型糖尿病族群的內皮細胞功能。第一種演算法為血管擴張指數,採用了RH-PAT的原理,計算壓脈帶加壓前、後一分鐘平均脈波振幅,第二種演算法為相似理論指標,是一種新的複雜的分析方法,它主要是觀察壓脈帶加壓前、後整體脈波振幅的變動。經過臨床實驗證明血管擴張指數與相似理論指標都能反應出健康人的內皮細胞功能。此外,相似理論指標比血管擴張指數更能有效的顯示出血糖與血管變化的關係。

The endothelium plays an indispensable role that regulating vascular tone and maintenance of blood pressure stability in blood vessels. When endothelial cells of vascular in the damaged cause endothelial dysfunction, increasing the formation of atherosclerosis, and produce cardiovascular diseases (hypertension, heart disease, diabetes and stroke). Many studies have suggested endothelial dysfunction is closely related to atherosclerosis and cardiovascular risk factors. Therefore, assessment of endothelial function can detect early atherosclerosis. To achieve early detection and early treatment.
In this study, we use two different algorithms to assess the endothelial function of health groups and type II diabetes. The first algorithm is the vasodilator index that using the RH-PAT is the principle and calculating the average pulse wave amplitude of cuff occlusion before/after one minute. The second algorithm is similarity theory, which is a new complex analysis method. We use it to observe fluctuations of pulse wave amplitude. In the result, both of vasodilator index and similarity theory can reflect the health of human endothelial function. In addition, similarity theory is more effective show the relationship between blood sugar and dilatation than vasodilator index.
