台灣末期腎病（End-Stage Renal Disease, ESRD）之發生率及盛行率皆位居世界之冠，其中≧65歲以上之患者更近半數；尤其對於老年期開始接受長期血液透析治療的患者而言，除了要面對老化所造成的生理功能衰退，還要承受透析治療可能帶來的生理不適或相關合併症，此時自覺老年階段應該要含飴弄孫，準備享福時，其內心的想法與需求，更應受到臨床專業人員的正視與關心。
本研究旨在探討老年末期腎病患者初接受長期血液透析治療之經驗。採質性研究設計，以立意取樣，自2010年5月至2010年8月，於北區某醫學中心血液透析室收案，運用深度訪談法及錄音筆收集十二位老年末期腎病患者初接受長期血液透析治療之經驗，受訪者年齡介於70至89歲，以內容分析法進行資料分析，發現老年患者從罹患慢性腎臟病至成為末期腎病患者之經驗可歸納出五個主題，分別為忽略身體警訊、關注腎病問題、擋不住的惡化、進入備戰狀態及面對透析生活，說明罹病至接受長期血液透析治療之時序性的動態過程。研究結果得知，老年慢性腎臟病患者會忽略身體警訊，主要受到不知與腎病有關、未感受嚴重性之因素影響；開始關注腎病問題後，進而採取飲食控制與持續追蹤之維護腎臟健康的行為，然而，在努力採取維護腎臟的實際行動後，卻發現自身疾病仍出現身體症狀加重，與腎功能指數不斷飆高的情形，因而被醫師告知隨時準備要接受透析治療的期間，其經歷與想法包括對洗腎相關資訊不甚瞭解、逃避與抗拒及等著被判刑。而在面對透析生活後，老年末期腎病患者歷經身體症狀困擾外，也要承受內心的煎熬，因此其內心深處出現期待過常人生活的聲音，同時，也會開始作人生的回顧與檢視，悟出其老年階段疾病感受之體會與自我省思。
藉由本研究結果，有助於深入瞭解老年末期腎病患者初接受長期血液透析治療之經驗與感受，可提供臨床專業人員於照護老年期開始接受長期血液透析治療患者之參考，以儘早幫助他（她）們適應晚年透析生活，進而提昇其生活品質。

Both the incidence and prevalence of ESRD in Taiwan are top in the world. Nearly 50% of ESRD patients are 65 years old or above. Usually the elderly would have expected to enjoy their pleasant family life. For those who are treated with long-term hemodialysis will suffer from discomfort and comorbidity through the entire therapy period other than the degenerative physical conditions, so the clinic professionals must pay more attention to the ESRD patients and tackle their fellings and care needs.
This study was based on the qualitative design. Purposive sampling was used to conduct in-depth interviews, which was implemented at the hemodialysis center of a teaching hospital located in northern Taiwan. The duration of data collection started from May 2010 through August 2010. Twelve ESRD patients aged 70 to 89 were interviewed. Five themes emerged from data analysis, including the negligence of physical warning, the concern about renal problem, continuous deterioration of renal function, preparing for hemodialysis, and facing the life of hemodialysis. The results showed the proceedings of incidence experience among the elderly ESRD patients. The study found that the negligence of body signals are normally resulted from lacking of self-awareness to its importance, lack of knowledge for renal disease. When patients understood their renal problems, even an action was taken such as dietary control and kept follow-up but renal function are still deteriorated. Finally they had to undergo hemodialysis treatment. They suffer from the experiences of various disturbance, the deteriorating physical symptoms and mental stress. They would start reviewing their life style and looking forward to having better quality of life.
In view of this study, it will help the health professionals to understand the experience and the feeling of those ESRD patients who are undergoing hemodialysis. This will assist them in improvement of ESRD patients' quality of life.
