外科部位感染會延長病人的住院天數，甚至造成病人的死亡。本研究目的希望探討改善措施介入前後，預防性抗生素使用情形與外科部位感染、住院天數及死亡等之相關性。
研究對象以北部某醫學中心2009年9月1日至2011年6月30日一般外科手術住院病人，共8,576人為整體個案，第一劑手術預防性抗生素使用cefazolin藥物者有3,255人，做為使用預防性抗生素之研究樣本。資料以描述性統計、卡方檢定、t檢定及邏輯斯迴歸分析。結果顯示：（1）手術預防性抗生素給藥時效，劃刀前60分鐘內使用預防性抗生素比率，由91%提升至95%；（2）手術時間大於4小時未使用第二劑抗生素的比率，從介入前94%下降至18%；（3）針對一般外科整體個案及使用預防性抗生素樣本統計結果發現：手術時間大於4小時、性別為男性、年齡、術前傷口分類及麻醉分類越高，其外科部位感染及死亡的風險越高；（4）整體個案介入措施前後在死亡率降低及外科部位感染、住院天數方面沒有呈現統計上的關聯；但使用預防性抗生素樣本在線性迴歸分析，平均住院天數降低0.06天（p<0.05），對於醫療資源耗用仍有其積極意義。
本研究結果可提供醫護人員正確使用預防性抗生素之參考，有助於醫療管理團隊建立正確預防性抗生素臨床使用之評估模式。

To begin with, Surgical site infections prolong hospitalization days, and even cause the patient's death. This study aims to investigate the differences between before and after implementing improvement measures intervention, and specifically, this study is to investigate the correlations between the usage of the surgical antibiotic prophylaxis , Surgical site infections and hospitalization days and death.
The object of this study is general surgical inpatients at the medical center in northern Taiwan, which are generally 8,576 people during September 1, 2009 to 2011 June 30.The first sampling from the medical information system database, only selecting the patients who are received the first dose of surgical antibiotic prophylaxis using cefazolin, are 3,255 people. Moreover, they are the samples of this study. The statistics of this study are including descriptive statistics, chi-square test, t test and logistic regression analysis. The results shows four dimensions. First of all, within 60 minutes before the draw knife , the rate of the surgical antibiotic prophylaxis administration followed by the time limitation is from 91 % to 95 %. Secondly, the rate of not using second dose of antibiotics prophylaxis when the operation time is greater than 4 hours is from 94% to 18%. The thirdly, male, older age , higher preoperative wound classification and anesthesia classification are suffering higher risk of surgical site infection and death rate when the operative time is greater than four hours. Fourthly, the surgical antibiotic prophylaxis in this study did not statistically show the relationships between mortality rate and surgical site infection. However, it is still in the positive way n medical resource consumption for it reducing the patient's average hospital stay of 0.06 days(p<0.05).
The results of this study can provide the reference of health care workers with the proper use of antibiotic prophylaxis, and it also contributes to the medical management team to establish the evaluation model of the proper use of antibiotic prophylaxis.
