臺灣博碩士論文加值系統

現代醫療強調以「人為中心」的醫學議題，發展醫療個人化，中醫則以個人化醫療見長，中醫可輔助現代醫學之臨床與研究，發展量表可做為臨床結合中西醫學的橋樑工具。故本研究之目的乃在建立中醫陰血虛體質量表並測試其信效度。
採用心理計量之量表發展模式與步驟，主要經四個步驟。步驟一：建立問項，由文獻搜尋及德爾菲專家法（N=38）產生陰血虛體質量表之問項與口語化題目。步驟二：先驅研究，確認「初問卷」符合民眾填答能力（N=81位）。步驟三：收案測試信效度，經醫學中心健檢門診收集20-60歲研究對象進行檢測（N=821位）。步驟四：效標關聯效度，以中醫師診斷為依據，由看診民眾(N=150位)填答問卷，測試量表與醫師間之ㄧ致性。
本研究完成之陰血虛體質量表包含19個問題。經探索性因素分析，萃取五個因素。五個因素分別命名為「頭部陰血不足」、「四肢陰血不足」、「腸道陰血不足」、「體表陰血不足」及「腹腔陰血不足」。整體量表內在一致性為0.848，五個向度的內在一致性值介於0.57~0.77，大部份題目的再測信度ICC值＞0.7。測量同時效標效度方面，中醫師診斷與量表得分達一致性結果，且個案量表得分與血色素值有一致趨勢。
本研究發展出五個向度19個題目之陰血虛體質量表，具有良好的信效度，可提供醫療人員臨床診療與研究之參考。

Background：Nowadays, the individual differences are emphasized in personalized medicine. Traditional Chinese medicine (TCM) which prescribes tailored treatment based on each patient’s different body constitution may provide new strategy to break the bottleneck of modern medicine (MM). Therefore, to integrate TCM with MM, an objective, reliable and rigorous diagnostic tool is necessary for the assessment of TCM constitution of each individual. The purpose of this study is to develop a Yin-blood-Xu Constitution Questionnaire (BCQ-), and test its reliability and validity.
Methods：The questionnaire of BCQ- was developed using a step-by-step approach: 1) to form the research team and select a panel of 38 experts who had both MM and TCM educational background and professional training for Delphi method; to generate questionnaire items from literature review and Delphi process, refine these items to be colloquially acceptable, and evaluate their face and content validity by Delphi process again; 2) to evaluate the difficulty of answering these questions by pilot study with 81 subjects whose age were between 20-60; 3)eight hundred and twenty one eligible participants, aged between 20-60, no disease onset in the past month, and receiving health examination in teaching hospitals were recruited to test the reliability and validity; 4) criterion- related validity was approval by 150 subjects.
Results：Nineteen out of the 22 items from BCQ- were developed. Exploratory factor analysis identified 5 domains that represent and correspond to the manifestations of the diminishing material level in 5 different body areas: Yin-blood-Xu in the head, four limbs, gastrointestinal tract, body surface, and abdominal cavity. The Cronbach’s coefficient alpha (α) ranged from 0.57-0.77 and the intraclass correlation coefficients were greater than 0.7 for most of the 19 questions. The concurrent validity for BCQ- in TCM physision and Hemoglobin showed consistent agreements.
Conclusion：Five domains with 19 items in BCQ- were developed. BCQ- is a valid and potentially useful tool for evaluating the Yin-blood-Xu constitution in clinical practices and researches.
