近年來由於社會和科技的快速進步,電腦已廣為在工作、生活、社群上應用,但過度依賴電腦的結果,就可能產生健康上的隱憂包括視力衰退及頸肩僵硬、腕隧道症候群、下背痛等,國內調查工作人員的肌肉疼痛情形傷害中也顯示頸肩疼痛佔較多比例。本研究目的為針對辦公室使用電腦作業者(1)探討以非侵入式方法評估頸肩症狀之可行性;(2)依問卷調查、理學檢查結果,瞭解頸肩症狀之風險因子;(3)進行簡易運動介入方案,比較分析改善頸肩症狀方案之有效性。
本研究選定46位辦公室工作者分為三組-實驗A組(n=19,職場簡易運動),實驗B組(n=13,自我運動者)、控制組(n=14),以問卷調查、理學檢查及雷射都卜勒血流計(LDF)進行非侵入式方法量測人體肩部之血液微循環流量(flux),於運動前後及六週後再進行前後測結果之比較分析,應用SPSS12.0軟體進行不同組別頸肩疼痛程度、身心健康值及微循環血流量之與相關分析。
本研究結果發現:(1)在過去一年中有>60%的受測者都曾有頸肩部疼痛的情形,且≧50%的人認為其頸肩疼痛症狀或全部是因為工作所導致;(2)年資、年齡與「頸部症狀」、「左右肩症狀」對個人的影響均呈正相關,但不顯著;休息時間長短對於「頸部、及左肩部位疼痛程度」呈負相關,但不顯著;(3)介入方案對實驗A組的頸肩部不適症狀有些微改善;(4)比較0週與6週的身體健康值(PCS)、心理健康值(MCS),三組0週時約在常態平均值範圍內(50±10)分,控制組與實驗A組的PCS平均得分在6週後有些微下降;反之,實驗A組在6週後MCS平均得分有升高情形。


In a fast-moving world, computers had been popularly used in many aspects of our life. However, over-use of computers may results in poor vision, carpal tunnel syndrome and low back pain, etc. In a national survey of muscle pain of workers in Taiwan, the ratio of neck-shoulder pain was higher than other types of pain. Focused on VDT(Visual Display Terminal, VDT) workers, the purposes of this study were to (1) investigate the feasibility of using a non-invasion device to evaluate the health condition of shoulder. (2) evaluate the risk factors of neck/shoulder symptoms using questionnaire and physical examination. (3) compare the effectiveness of an intervention strategy for the prevention of neck/shoulder symptoms.
46 VDT workers were selected and divided into three groups – “Experimental group A” (n=19, short-exercise on-site), “Experimental group B” (n=13, self-exercise off-site), and the control group (n=14). Questionnaire survey and physical examination were conducted. Laser Doppler Flowmetry (LDF), a non-invasive method, was used to measure the microcirculation flux of human shoulder before and after short-exercise, as well as measure the microcirculation flux. SPSS12.0 software was applied for data analysis. Neck-shoulder pain levels, physical and mental health survey and LDF measurements for different groups were compared.
It was found that: (1) more than 60% of the VDT workers reported neck-shoulder pain, and 66.7% recognized that neck-shoulder pain was attributed by VDT work;(2) work duration, age, neck symptoms, “left and right shoulder symptoms” were positively correlated with “personal impact” marginally; and the “neck, left shoulder pain” was negatively correlated with the break time marginally; (3) the intervention program has slightly positive effect on the shoulder and neck symptoms of the “Experimental group A”; (4) the physical health value (PCS), mental health values (MCS)of the three groups at 0 weeks were within the average range (50 ±10) of the norm;the average PCS score of the control group and “Experimental group A” score slightly declined after six weeks; on the contrary, the average MCS score of “Experimental group A” significantly increased after 6 weeks.
The risk factors of neck-shoulder symptoms for VDT workers including age, work duration, break time during work and exercise habit, etc. LDF seemed to be a promising tool for early evaluation of shoulder symptom. It is suggested that short-time, group exercise seems have positive effect on the prevention of neck-shoulder symptoms for VDT workers, especially performing extended-whole body exercise.
