背景:呼吸器依賴病童對家庭產生了重大的改變,影響家庭功能的運作,轉而引發家庭韌力。且其家庭成員面對衝擊處理的能力,也取決於面對壓力掌控、家庭資源運用的能力。本研究旨在探討影響呼吸器依賴病童家庭功能之家庭韌力、家庭成員壓力掌控與家庭資源的相關性,協助醫護人員提供以家庭為中心的實務運用。目的:本研究主要目的在探討呼吸器依賴病童家長之壓力掌控、家庭韌力、家庭資源與家庭功能的現況。並進一步了解背景變項對家庭之壓力掌控、家庭韌力、家庭資源、家庭功能之影響,分析家庭背景變項、壓力掌控、家庭韌力、家庭資源、與家庭功能之關係及預測。方法:研究採橫斷式、描述相關性研究設計,以立意取樣方式採問卷調查法,以呼吸器依賴病童家庭為本研究對象。研究者採用五種結構式問卷(家庭資料、壓力掌控量表、家庭韌力量表、家庭資源量表、家庭功能量表)蒐集量化資料,得有效問卷100 個病童家庭。結果:研究結果顯示(1)呼吸器依賴病童家庭的家長現況具有良好的壓力掌控;病童家庭展現出良好的韌力;擁有足夠的家庭資源;及良好的家庭功能。(2)病童年齡、呼吸器使用年資、家庭結構、父母親年齡及社經地位在研究變項中並無顯著差異。病童性別、疾病因素、長期照護場所、主要照顧者、呼吸器使用情形及時間、每月支出費用、就學狀況、家中第一個孩子年齡、家庭生活壓力、父母親婚姻狀況有顯著差異。(3)相關分析結果發現,壓力掌控與家庭韌力有正相關;壓力掌控與家庭資源有正相關;壓力掌控與家庭功能有正相關;家庭韌力與家庭資源有正相關;家庭韌力與家庭功能有正相關;家庭資源與家庭功能有正相關。(4)以多元迴歸分析結果顯示:自尊與溝通、家庭健康資源、家庭溝通與經濟社會指標為整體家庭功能的預測變項可解釋75.5%之總變異量。結論:研究結果對影響呼吸器依賴病童家庭功能之家庭韌力因素提供科學性知識,且建議以促進家庭發展之家庭為中心之護理措施。
The Family Resilience Factors Affect on Family Function of
Ventilator-dependent children
Abstract
Background: Ventilator-dependent children have a great impact on their families, affecting family functions and further leading to family resilience. Family members’ ability in dealing with impact issues depend on their ability in mastery of stress and use of family resources. The current study focuses on the correlation of the effect of family resilience, family member mastery of stress, and family resources on the family function of ventilator-dependent children to provide nursing personnel
with family-centered practice. Purpose: This study explores the current situation of the mastery of stress, family resilience, family resources, and family function of parents of ventilator-dependent children. The purpose of this is to further understand the effect of the background variables on the mastery of stress, family resilience, family resources, and family function and analyze the relationship and prediction of background variables, mastery of stress, family resilience, family resources, and family function. Method: A cross-sectional descriptive correlational design was adopted in this study using purposive sampling and questionnaire surveys. Families of ventilator-dependent children were recruited as participants. Five types of structured questionnaires were employed including family information, mastery of stress scale, family resilience scale, family resources scale, and family function scale for quantitative data collection. A total of 100 valid questionnaires were retrieved.
Results: The results showed that (1) based on current conditions, the parents of ventilator-dependent children performed well in terms of mastery of stress. while families of patients had good resilience, adequate family resources, and good family function; (2) there was no significant difference in the age of children and years of using ventilators in the variables, family structure, age of the parents, socioeconomic status. there was significant difference in the in the gender of children and diseases, Long-term care places, Primary caregivers, ventilator use status and use time, family with a monthly expenditure, receiving education status, families whose eldest child age, family inventory of life event, Parents'marital status. (3) correlation analysis results indicated a positive correlation between mastery of stress and family resilience, mastery of stress and family resources, mastery of stress and family function, family resilience and family resources, family resilience and family function, and family resources and family function; and (4) multiple regression analysis results showed that self-esteem and communication, family health resources, family communication, and economic and social indicators are the predictive variables of the overall family function with a total of 75.5% variance explained.
Conclusion: The results of this study provides scientific knowledge for the family resilience factors that affect the family function of ventilator-dependent children and suggests family-centered nursing interventions that are able to facilitate the development of families.
