本研究於100年8月及12月在淡水河系之新店溪與大漢溪採樣,以固項萃取法搭配液像層析串聯質譜儀,分析水中14種醫院常使用藥物之殘留濃度,並探討其空間分佈。分析結果這14種醫療殘留藥物在淡水河流域中的濃度範圍如下:acetaminophen , nd-547.17 ng/L; amlodipine , nd-3.24 ng/L; atorvastatin, nd-6.44 ng/L; bezafibrate, 0.22-26.81 ng/L; caffeine, 41.38-3455.17 ng/L; carbamazepine, nd-27.24 ng/L; gemfibrozil, 3.66-932.76 ng/L; irgasan, nd-959 ng/L; naproxen, nd-92.58 ng/L; nimesulide, nd-57.74 ng/L; pentoxifylline, nd-22.43 ng/L; roxithromycin, nd-2.29 ng/L; sulfamethoxazole, nd-76.80 ng/L; valsartan, nd-924.15 ng/L,這14種醫療殘留藥物以caffeine的濃度最高為µg/L等級,其它醫療殘留藥物的濃度大都為ng/L等級。而acetaminophen和gemfibrozil的濃度高於歐美先進國家河水中的濃度調查,此兩種藥物為我國藥物銷售榜上的熱門藥物。

在8月與12月的調查中,多數藥物的濃度呈現明顯差異,12月的acetaminophen、amlodipine、naproxen和nimesulide1等殘留藥物濃度較8月高出許多,且上游區域濃度較高,顯示有污染源輸入,一般而言,這些殘留藥物的濃度大都向河口遞減。

The river and estuarine waters collected from the Danshuei River and its tributaries, namely the Hsintienand the Dahan River. The concentrations of fourteen pharmaceutical compounds, which are the common prescription in Taiwan hospital, in the water samples were analyzed by the solid-phase extraction with high performance liquid chromatography tandem mass spectrometry. The concentration ranges of these analyzed pharmaceutical compound were as follows: acetaminophen , nd-547.17 ng/L; amlodipine , nd-3.24 ng/L; atorvastatin, nd-6.44 ng/L; bezafibrate, 0.22-26.81 ng/L; caffeine, 41.38-3455.17 ng/L; carbamazepine, nd-27.24 ng/L; gemfibrozil, 3.66-932.76 ng/L; irgasan, nd-959.00 ng/L; naproxen, nd-92.58 ng/L; nimesulide, nd-57.74 ng/L; pentoxifylline, nd-22.43 ng/L; roxithromycin, nd-2.29 ng/L; sulfamethoxazole, nd-76.80 ng/L; and valsartan, nd-924.15 ng/L. The concentration of caffeine was in the µg/L level and was the highest among the studied pharmaceutical compounds. The concentrations of the other pharmaceutical compounds were in the ng/L level. The concentrations of acetaminophen and gemfibrozil found in the Danshuei River were generally higher than those reported in the European and American countries.

The concentrations of the most studied pharmaceutical compounds varied during the August and December surveys. The concentrations of acetaminophen,amlodipine,naproxen, and nimesulide1 obtained in December were higher than those of August, and the higher values appeared in the upper stream, suggesting a pollution source input to the upper stream. The concentrations of the studied pharmaceutical compounds generally exhibited decrease with seaward, which is the dilution effect of seawater.
