目的:探討踝關節扭傷患者在穿著踝關節護具由步態行走及上階梯之生物力學參數之變化情形。研究對象:以八名處於恢復期之青少年男性腳踝扭傷患者(年齡:14.88±0.64歲、身高169.81±3.44公分、體重60.16±11.96公斤重)為受試對象。方法與步驟:使用一台Mega Speed Ms30K高速攝影機(100Hz)、AMTI測力板(1000Hz)兩台與Biovision肌電儀(1000Hz)以同步的方式擷取步態之生物力學與肌電訊號參數。踝關節護具係以綁帶型踝關節護具(Lace-up ankle brace)和兩側支撐型護具(Air stirrup ankle brace)兩種護具來定義之。所得實驗參數使用SPSS for Windows 18.0套裝軟體以單因子重複量數變異數分析進行統計分析(α=.05)。結果:一、穿著踝關節護具於步態行走與上階梯動作均會限制踝關節背屈且有達到統計上顯著水準(p<.05)。在步態行走時將減少踝關節屈與背屈角位移3-4∘左右,在上階梯動作時則會減少4-5∘左右。二、在地面反作用力部分綁帶型護具不論步態行走或上階梯,其前後分力峰值或垂直分力峰值以及負荷率均有達到顯著差異(p<.05) 明顯小於控制組。而支撐型護具則僅有上階梯動昨時之前後分力峰值與垂直分力峰值有達到顯著差異(p<.05)。三、肌電部分僅有綁帶型護具於步態行走與上階梯動作制動期之平均肌電振福,股直肌的肌肉活化達顯著差異(p<.05)。結論:穿戴踝關節護具能限制踝關節活動範圍的角度、而使制動期期垂直分力與水平分力峰值減少,進而達到預防踝關節扭傷的效果,且以綁帶型踝關節護具效果較佳,為了減輕下肢負荷,建議踝關節扭傷患者於恢復期能穿著護具,進而避免二次扭傷的頻率。
Purpose: to investigate the variance of biomechanic parameters of gait and stair climbing upon wearing the ankle brace of patients with a sprained ankle. Subjects: 8 teenaged males with a sprained ankle at the recovery stage were recruited (mean age 14.88±0.64 year-old; height 169.81±3.44 cm; weight 60.16±11.96 Kg).Materials and method: Biomechanic and muscles’ electrical activities’ parameters were obtained synchronously by a Mega Speed Ms30K high speed camera (100Hz), two ATMI force platforms (1000Hz), and a Biovision electromyography (EMG) system. Study was applied the lace-up ankle brace and air stirrup ankle brace as the definitions for ankle braces. One-way Repeated Measures were performed to statistically analyze the effects of various parameters by SPSS for Windows 1.8 software (significant difference α=.05). Results: 1. Ankle brace wearing has significantly limited the dorsiflexion in movements of gait and stair climbing (P<.05). The displacement of dorsiflexion decreases about 3-4 degrees in gait, and 4-5 degrees in stair climbing. 2. The vertical, horizontal ground reaction and the average rate of weight acceptance forces have significantly smaller than the control groups when wearing the Lace-up ankle brace(P<.05) in the movements of gait and stair climbing. For the Air stirrup ankle brace, significant differences (P<.05) only have shown in the vertical and horizontal ground reaction during the stair climbing. 3. The muscle activity of rectus femoris showed a significant difference on the amplitude in the two movements of the gait and stair climbing (P<.05). Conclusion: Wearing the ankle braces can confine the angles of ankle joint movement and reduce the horizontal and vertical ground reaction peak forces during the braking phase to achieve the effect of the sprained ankle prevention, and the Lace-up ankle brace shows a better effect. To relieve the burden for the lower limbs, we suggest patients with a sprained ankle wearing the ankle brace during the recovery stage to avoid the second time sprain.
