腦損傷傷患可分為外傷性顱出血與自發性顱內出血,腦損傷所引起腦組織損傷常會導致嚴重的後遺症以長期預後之恢復居多,對於醫療資源的耗損也相當多,但國內針對腦損傷預測病人預後意識恢復之相關研究較少。腦部是精密器官腦部經外科手術具有高度之危險性,病人常在手術前對疾病之不確定感,以及手術後病人意識恢復頇面對長期復健治療,這些對病人及主要照顧者而言都極具壓力的。故本研究的目的在預測腦損傷病人六個月內清醒之相關指標,採回顧病例方式進行研究,以中區區域教學醫院之腦損傷接受手術之病人共185人為研究對象,以資料探勘技術統計方法進行資料分析。
研究發現:以貝式、類神經網路、決策樹、邏輯斯迴歸四種演算方法比較不同母群體因素、不同研究個案排除條件因素、不同訓練樣本因素與不同驗證資料因素對於預測模型評估結果的影響,預測腦損傷病人六個月意識恢復程度探討相關因子相對之重要性。
研究結果發現:1.預測病人意識恢復與年齡、昏迷指數及血糖值皆有顯著相關,年齡愈高、昏迷指數愈低、血糖值愈高,其意識恢復明顯較差(p<0.001)。2.預測病人意識恢復與性別、糖尿病史、高血壓病史、診斷、白血球較無顯著之差異。結論:昏迷指數可以有效的評估腦損傷病人之意識恢復程度,並且將對於意識恢復情況的預測具準確度,將結果可提供臨床醫護人員對腦損傷病人六個月清醒與否預測之指引,期望透過精確的病情進展評估,對腦損傷病患經接受醫療照護的政策發展有所貢獻。
Typical brain injuries are due to traumatic cranial hemorrhage or spontaneous intracranial hemorrhage. The impact of brain trauma to each patient is quite diverse, from a vegetative state to a near-complete recovery from the physical motor function disorders to the invisible cognitive behavioral dysfunction. Currently, there is limited research available in the area of long-term prognosis for patient with brain injury.
The purpose of this study is to develop a long-term prognosis for patient with brain trauma within the six months period. 185 patients from the central regional teaching hospital were selected for this study. The age of the patient, gender, blood sugar level, blood pressure, and white blood cell count are determined to be the critical indicators whether the patient will regain full consciousness in six months of not. The findings of this study will be beneficial and can be used as a guideline by the medical stuff to assess the probability of the patient’s chance of recovery from the brain injury.
