隨著年齡的增加，個體的功能便不斷地衰退，當衰退到一個程度就無法生活及參與，此時就必然出現了長期照護的需求。掌握高齡者或身心障礙者其無法涵括或尚未明朗化的照護需求時，以預防身體功能退化或失能，即在邁入長期照護之先，取得先機-早期發現其潛在性長期照護之風險，並有所為。本研究採橫斷式研究設計，對象以台灣北部某縣市65歲以上參與健檢之19,190位老年人，首次以公共衛生篩檢嘗試搭配日本介護預防篩檢表之引用，嘗試找出隱蔽於社區中之潛在性長照需求風險者，研究觀測結果進行描述性及同步迴歸分析，嘗試從不同向度探討潛藏之長期照護需求風險。研究觀測顯示：受訪者平均年齡74.4±6.8歲(67.6 to 81.2)，整體性具潛在長期照護需求風險者共2,725人，佔14.2%，其中綜合性潛在長照需求風險面向切入者共453人佔2.4%，依潛在風險向度切入之排序分佈如下：具活動/運動力潛在長照需求風險者共1,225人，佔6.4%；膳食營養潛在長照需求風險者共234人，佔1.2%；具口牙潛在長照需求風險者共601人，佔3.1%；最後是可能具憂鬱/鬱卒長照需求風險者共212人，佔1.1%。研究觀測，發現變項中以社區65歲以上老年人之社會人口學特性中，性別、年齡、教育程度、經濟狀況、婚姻狀態、宗教信仰、家庭結構與整體性潛在長照需求風險有顯著相關。女性在整體性之潛在照護風險高於男性；隨著年齡的增加，活動力潛在長照需求風險即增加；婚姻狀態與口牙潛在長照需求風險相關、經濟狀況與憂鬱/鬱卒潛在長照需求風險相關；至於罹病狀態與整體性之潛在長照需求風險面向，並無顯著相關。樣本整體解釋力R2僅為11.8%，其結果不如預期。本研究係以日本介護預防篩檢表所搭配之初步篩檢結果，並首次透過公共衛生篩檢結合潛在長期照護需求或風險的概念嘗試，提供初步研究之結果，顯現多向度切入之潛在性長期照護需求風險資訊，由於干擾因素眾多，樣本並非充裕，故研究結果僅供參考之用，期望未來更多相關研究支持並探討之。

The long-term care(LTC) needs/risks , accompanied with functional decline on activity & participation(A & P), increased with age. In order to gain access LTC needs/risks potentially in advance, we tried to detect latent LTC needs and coped with functional decay or disability. Under cross-sectional design, the study aimed to conduct screening by Kaigo Yobo checklist from Japan on the elderly in northern Taiwan County/City, tried to detect those with LTC needs in potential via various domains by description and regression analysis. Out of 19,190 samples whose age from 67.6 to 81.2 (74.4±6.8), overall 2,725 (14.2%) with the risk of LTC in potential, and among them prioritzedly, 453 (2.4%) via combined domain, 1,225 (6.4%) via mobility domain, 234 (1.2%) via diet & nutrition domain, 601 (3.1%) via oral & teeth domain, 212 (1.1%) via depression domain. Ultimately, LTC in potential be supposed to with positive correlation with significance among gender, age, education, income, marital status, religion, and family structure, so female & elderly more risky; furthermore, gender for combined domain, age for mobility domain, marital status for oral & teeth domain, income for depression domain, but not the morbidity status for LTC in potential whatever. Out of expectation, the R2 has been 11.8% only, our workup just presented a pilot outcome with brand new Kaigo Yobo checklist for new century under current public health framework and delivery system, indeed it help exploring some LTC needs in potential among community elder dwellers, however, the related resource and cooperation for conduction not very sufficiently. Therefore, the study can be regarded as the reference basis for LTC policy, organizing, operating and delivery, more similar or related studies are mandatorily.
