正子分子影像檢查(FDG-PET)是目前診斷癌症、心臟病及神經精神疾病等病
症的重要診斷工具。然而,現今國內各家醫院多只有一台正子分子影像掃描儀,
隨著罹癌人數上升、健保給付範圍放寬以及民眾健檢觀念越趨健全,正子分子影
像檢查的需求量因而倍增。在臨床上,檢查時間少則 2-3 小時,多則 5-6 小時。
檢查的排程、等候的時間與機器使用率,關乎受檢者的滿意度及醫療院所的成本
與利潤。
本研究透過精實工具中之價值流程圖(VSM)的使用,找出增值時間和非增值
時間。資料收集 2013 年 8 月,一個月間所有受檢者的檢查時間紀錄表,統計
分析各醫師在檢測各部位的價值流程圖。利用問卷調查了解受檢者在各站點等對
診候時間的滿意度,找出可控制因素和不可控制因素,以釐清各因素伴隨增值或
非增值時間。
Positron molecular imaging (FDG-PET) is an important diagnostic tool for the
diagnosis of cancer, heart disease and neurological disorders and other mental
disorders. However, the domestic hospitals usually have one FDG-PET scanner only .
With increased numbers of people suffer from cancer, relaxation of the scope of
national health insurance and more robust concept of public health screening, the
demand of FDG-PET has been doubled. Clinically, the procedure takes at least 2-3
hours up to 5-6 hours. The Inspection schedule, waiting time and machine utilization
will affect the satisfaction of patient and the profits of medical institutes.
In this study, one of the lean tools, value stream map (VSM), is applied to
identify value-added and non-value added time. From August 1, 2013 to August 31,
2013, patients visited at the duration whose time records of examinations were
retrieved. Those records were analyzed according to doctors and parts of patients’
body to construct VSM. A questionnaire was used to reveal patients’ satisfaction in
each station about waiting time. Meanwhile, controlled factors and uncontrolled
factors were found and value-added or non-value added time associated with each
factor was clarified.
