背景與目的:近年來人們對智慧型手機的使用頻繁程度和依賴越來越高,但智慧型手機的研究明顯不足,尤其缺乏不同姿勢和長時間使用所造成影響的研究。本研究的目的為分析不同使用方式下,長時間連續執行智慧型手機活動對疼痛和脊椎姿勢的影響。研究方法:本研究納入條件為20-30歲健康成年人為受試者。以桌子和大腿使用方式執行30分鐘智慧型手機打字活動,分析疼痛狀況和脊椎姿勢的變化。以視覺疼痛量表(Visual analog scales, VAS)測量疼痛。以三度空間動作分析系統測量脊椎姿勢,共測量四種角度包含頭部角度(Head angle, HA)、頸部角度(Neck angle, NA)、顱頸角度(Cranio-cervical angle)和骨盆角度(Pelvic angle)。也測量基本資料和使用智慧型手機的習慣和狀況。結果:不論桌子或大腿使用方式,最嚴重和使用後的疼痛在頸、肩及腰部都明顯增加(p < 0.05),尤其頸部最嚴重的大腿使用方式(VAS = 4.27)比桌子使用方式(VAS = 2.77) 疼痛更嚴重(p < 0.05)。兩種使用方式,頭部、頸部和骨盆角度進程姿勢都明顯改變(p < 0.001)。頭部屈曲越多時,肩部疼痛會越多(r= 0.37-0.47)。每天使用手機時間越久的人,肩部疼痛越多(r= 0.39)和骨盆角度越後傾(r = 0.40)。結論:執行30分鐘智慧型手機打字活動會造成頭頸部屈曲角度的不良姿勢,也產生頸肩腰的疼痛問題,並且

依靠大腿使用方式比桌子使用方式更為嚴重。建議使用智慧型手機時盡量選擇桌子使用方式執行,經常提醒自己減少頭頸部屈曲的動作,應可減少疼痛情況。

Background and purpose: People used the smartphone frequently, but the studies for the smartphone are insufficiency. The purpose of this study was to analysis the effects on pain and angle of the spine by prolonged use of the smartphone. Study methods: The study included healthy adults aged from 20 to 30 year old. Participants were asked to type on smartphone for 30 minutes in two trials, leaning on the desk and lap. Participants were assessed pain severity (the neck, shoulders and lumbar regions), spinal angle (the head, neck, cranio-cervical and lumbar angles) and the habits of using smartphone. The pain severity was evaluated by the Visual Analog Scales (VAS), and the spinal angle were measured by the 3-D motion analysis. Results: The VAS of neck, shoulders and lumbar were increased in two trials (p < 0.05), and more severe on the neck in leaning on the lap (VAS = 2.77 and 4.27 for desk and lap, respectively, p < 0.05). The angle of head, neck and pelvis were significantly changed from the start to the end of experiment in both trials (p < 0.001). The degree of head flexion was associated with the VAS of shoulders (r = 0.37-0.47). The longer time a subject spending on using smartphone per day was higher VAS score on shoulders (r = 0.39) and more pelvic posterior tilt (r = 0.40). Conclusion:

Typing on smartphone for 30 minutes induces greater head and neck flexion angle. Furthermore it increases pain on multiple regions, and leaning on the lap caused more pain than leaning on the desk do. Therefore, we recommend that leaning on the desk instead of on the lap when using smartphone, and remind yourself to change the posture frequently to reduce the pain.
