論文名稱： 膝關節置換術後五年之健保資源耗用分析-中西醫治療比較

背景：根據行政院衛生署中央健康保險局（2011）資料顯示，雖台灣老年人口比例於2007年才剛超過10％，比同期間歐美日等已開發國家的老化程度還低，而且目前人口老化對醫療費用的影響尚未明顯，但是未來等到台灣高峰期人口（大約出生在1950-1990年期間）逐漸步入退休年齡之際，可以預見相關健康保險資源耗用的情形必將逐漸突顯。預估到了2060年，65歲以上高齡人口占總人口比率將由2009年10.6%攀升至2060年41.6%。隨著台灣老年人口越來越多，有關膝關節退化的預防及膝關節置換術後龐大的醫療費用也將逐漸成為國家健康保險的重大負擔。因此，有效協助老年人口之膝關節患者進行有效的治療及保健，已經成為醫療機構不可忽視的重大任務。
目的：利用健保資料庫，針對台灣膝關節病患進行長期追蹤，探討中西醫治療的膝關節置換術後五年之健保資源耗用分析。
研究方法：本研究使用健保資料庫2005年版百萬歸人檔資料，選取2000~2006年的膝關節術後病患為樣本，並排除不符合本研究限制的部份，再針對樣本之每位病患追蹤五年，以SAS 9.3版軟體進行描述性及推論性統計。描述性統計以平均數及標準差、樣本數與百分比呈現，推論性統計使用Student T-test、Chi-square、Linear regression進行檢定，探討醫療利用型態對病患膝關節置換術後五年之健保資源耗用分析。
結論與建議：本研究發現膝關節患者術後選擇中西醫治療比率顯著高於單選擇西醫治療，顯示我國膝關節患者進行膝關節置換術後選擇採用中西醫治療及調養已成為就醫趨勢。依此，建議未來將可發展膝關節置換術後成效之相關實證醫學以及成本效益之研究，同時探討中西醫治療之有效性，以避免醫療資源耗用。

Abstract
Title of Thesis: Analysis of Resource Consumption for National Health Insurance within 5 Years after Total Knee Replacement-A Comparison between Western Medicine and Chinese-western Medicine Combined Treatments.

Background: According to the National Health Insurance Administration (2011), Taiwan elderly population in 2007 was just over 10%, less than which of Europe and other developed countries, and the health care costs of elderly population were still unapparent. However, as the peak population (born around the period of 1950 and 1990) gradually going into retirement, it is foreseeable that health insurance resources consumption will be more and more obvious. Estimate until 2060, the elderly population rate of over 65 years old in Taiwan is anticipated rising from 10.6 percent in 2009 to 41.6 percent. Accompanying the speedy growth of Taiwan''s elderly population, huge medical expense of degeneration of the knee joint would gradually become a major burden on the National Health Insurance (NHI). Therefore, to effectively assist elderly patient for effective treatment of knee joint degeneration and healthcare after Total Knee Replacement (TKR) has become a critical task of medical institutions.
Purpose: Utilize NHI database for tracking TKR patients to analyze resource consumption of Western medicine and Chinese-western medicine combined treatments within five years after TKR.
Method: This Study used NHI database 2005 version which select TKR patient from 2000 to 2006.Each entries were followed 5 years. SAS 9.3 were used for analysis.
Conclusion: This study found that more TKR patients significantly opted Chinese-Western combined treatment than Western medicine treatment. The result demonstrated the tendency towards Chinese-Western combined treatment has become patients'' main choice after TKR to nurse and take care of their knees health. Accordingly, to prevent excessively medical resource consumption, further study about evidence-based medicine and cost-effectiveness on TKR postoperative are recommended.
