背景與目的:偏頭痛是一種會出現反覆輕度或重度頭痛的慢性疾病,發作時不只會影響生活品質、更可能造成生產力下降。國外已有許多文章證實偏頭痛與睡眠障礙、過敏性鼻炎、氣喘是有相關的,但國內對此議題的研究還不多,故本研究旨在探討偏頭痛與睡眠障礙、過敏性鼻炎、氣喘之相關性。
方法:本研究研究資料使用2000~2013「衛生福利部衛生福利資料科學中心」之B組「兩百萬人抽樣檔」。以2001-2012年門診診斷為偏頭痛,三次以上,作為研究母群體;並排除2001年的前一年(2000年)曾被診斷偏頭痛之病患。本研究使用SAS9.4版進行分析,並使用傾向分數配對法,來降低兩組間的特性差異。以一般病患為對照組,分析偏頭痛與睡眠障礙之相關性。
結果:有偏頭痛的患者罹患睡眠障礙是無偏頭痛的患者罹患睡眠障礙的2.62倍(95% CI=2.42-2.83;P<0.0001);偏頭痛的患者罹患過敏性鼻炎是無偏頭痛的患者罹患過敏性鼻炎的2.22倍(95% CI=2.04-2.43;P<0.0001),用藥品項對罹患過敏性鼻炎是有相關的( HR=0.84);有偏頭痛的患者罹患氣喘是無偏頭痛的患者罹患氣喘的1.27倍(95% CI=1.09-1.48;P=0.002),使用非特定治療用藥、特定治療藥物、用藥品項四者對罹患氣喘是有相關的(HR=1.99 vs. HR=2.60 vs. HR=0.57)。
結論:研究結果顯示,睡眠障礙、過敏性鼻炎、氣喘確實與偏頭痛是有相關的,偏頭痛的用藥型態也會影響睡眠障礙、過敏性鼻炎以及氣喘。
Background:Migraine is a chronic disease that will occur repeatedly mild or severe headache. When the migraine occur, It will affect their emotions, quality of life, more likely to cause decreased productivity. Some articles revealed that migraine related to sleep disorders, allergic rhinitis and asthma, there are few studies reported correlation between migraine and sleep disorders, allergic rhinitis and asthma in Taiwan. The correlation between the correlation between migraine with sleep disturbance, allergic rhinitis and asthma.
Method:This research was was a retrospective cohort study using the 2000-2013 2,000,000 ”Registry for beneficiaries” Database, a nationally representative database established by the National Health Research Institutes. The study group was diagnosed with migraine as a migraine headache for more than three times in 2001 - 2012, and excluded patients who had been diagnosed with migraine in the previous year (2000). This study was analyzed using SAS 9.4 and the use of a tendency score pairing method to reduce the difference in characteristics between the two groups. The general patient as the control group, analysis of migraine and sleep disorders related.
Results:Patients with migraine and sleep disorders were 2.62 times of sleep disorders in patients without migraine (95% CI=2.42-2.83;P<0.0001). The use of specific treatment of those who have sleep disorders are not using specific treatment of 1.19 times. Patients with migraine and allergic rhinitis were 2.22 times of allergic rhinitis in patients without migraine (95% CI = 2.04-2.43; P <0.0001). The use of both specific medications and medications is associated with allergic rhinitis (HR = 1.25 vs. HR = 0.84). Patients with migraine and asthma were 1.27 times of asthma in patients without migraine (95% CI = 1.09-1.48; P = 0.002). The use of non-specific therapeutic agents, specific therapeutic agents, prophylactic and pharmaceutical items was associated with asthma (HR = 1.99 vs. HR = 2.60 vs. HR = 1.54 vs. HR = 0.57).
Conclusion:We found that migraine related to sleep disorders, allergic rhinitis and asthma, prescription patterns of migraine affects sleep disorders, allergic rhinitis, asthma.
