臺灣博碩士論文加值系統

世界衛生組織國際健康功能與身心障礙分類系統(ICF)出版後，推出了以ICF架構參考的Mini-ICF-APP，用來評估精神疾病患者參與活動或能力的損傷。Mini-ICF-APP已被證明是有信效度的評估工具，但是尚未在臺灣使用過，本研究目的是要進行該量表的實務驗證，探討社區精神病患的社會功能現況以及建構SEM模型。
本研究為量化研究，採立意取樣，對象以在南部某精神科專科醫院門診且符合DSM-5診斷之社區精神疾患者，有效樣本200位，其中50位於三個月後再度評估，研究工具為英文版Mini-ICF-APP，透過訪談評估社區精神疾患者之社會功能現況。
研究結果顯示，有九成社區精神疾患者的社會功能落在中度失能以下，未成年發病、學歷較低者，整體失能越高，不過生病年數對社會功能並沒有直接影響。整體而言，社區精神疾患者的「自發性活動」、「自我照顧」及「移動性」普遍無損傷，但是「靈活性」、「能力」及「自信」失能較高。經過三個月後重新評估發現，在住宿型精神復健機構接受精神復健治療的患者中，思覺失調症患者比其他診斷患者的社會功能有明顯進展。學歷、工作現況、身心障礙證明等級與Mini-ICF-APP有顯著差異，而「靈活性」、「與他人接觸」、「團體整合」及「耐力」對工作有預測力。刪除「維持常規」且解構為三個潛在構面後，其模型更精簡且模型配適度更佳，是更適合整體社區精神疾患者的在地化模型。因此，Mini-ICF-APP是適用臺灣社區精神疾患者社會功能的評估工具，以提升社區精神疾患者社會功能之處遇方向。最後，依此研究結果對實務工作者、精神復健模式及政府等提出相關建議，以做為實務及政策之參考。

After publication of the WHO International Classification of Functioning Disability and Health (ICF), the Mini-ICF-APP was derived and to assess limitations of activities and participation in Mental Illness Patients. The Mini
-ICF-APP has been validated, but this has not been used in Taiwan. The purpose of this study is to validate Mini-ICF-APP and to construct SEM model. The study was quantitative research, purposive sampling. Two hundred patients were consecutively recruited from a psychiatric specialized hospital of South Taiwan.The diagnosis was assigned on clinical grounds by psychiatrists working in the hospital using DSM-5 criteria. 50 patients were
interviewed on two occasions to ascertain test-retest reliability. Research scale is an English version of the Mini-ICF-APP.
Results：The social function of 90% of the community mental illness patients fell below moderate disability. Lower education patients and on set in the minor have lower social function. However, the number of years of illness does not have a direct impact on social functioning. In general, spontaneous activities, self care and mobility are no impairment generally. Flexibility, competency and assertiveness are moderate disability. After mental rehabilitation treatment, social function of patients with schizophrenia had significant progress. Flexibility, contact with others, group integration and endurance have a predictive power of work. Delete ‘adherence to regulations ’, and deconstruction of the three potential factors, the model is more streamlined and the model is more appropriate localization model. Therefore, the English version of Mini-ICF-APP is a appropriate instrument for use in
evaluating social function in mental illness patients of Taiwan community. Finally, based on findings of this study, the ample and concrete suggestions and references of mental rehabilitation model are offered for related
organizations.
