本研究之目的為使用台灣版華語噪音下語句聽辨測驗(Mandarin Hearing in Noise Test in Taiwan, MHINT-T),評估單側感音神經性聽損者於安靜環境下的語句聽辨閾值,以及各噪音源位置下語句聽辨率為50%時的訊噪比(signal to noise ratio, SNR )。此外,並使用成人聽覺表現量表(Hearing Handicap Inventory for the Adults, HHIA),評估單側聽損者自覺帳障礙程度,並評估客觀語句聽辨測驗及主觀問卷量表間的相關性。

本研究經由網路招募的方式,蒐集共25位平均年齡為31.4歲的聽力正常者為對照組,16位平均年齡為38歲的單側感音神經性聽損者為實驗組。
研究結果呈現出單側聽損者於安靜環境時的語句聽辨閾值為26.0 dBA,標準差為2.7 dB。噪音在前播放、噪音在優耳側播放以及噪音在劣耳側播放時,語句聽辨率為50%時的SNR分別為-4.6 dB、-4.3 dB、以及-8.7 dB,標準差分別為1.0 dB、1.8 dB、以及2.1 dB。實驗組與對照組之間的上列情境測試結果均呈現顯著差異(p < 0.05)。單側聽損者於HHIA中的總分、情緒得分,以及社交得分,其平均表現分別為33.3、18.3、以及15.0分。16位單側聽損者中分別有4位屬於自覺沒有障礙、9位屬於自覺輕度障礙,3位屬於自覺重度障礙,其百分比約25%、56%,以及19%。使用斯皮爾曼等級相關係數(Spearman’s correlation coefficient)分析單側聽損者噪音下各種情境(noise composite)之語句聽辨表現與HHIA中自覺障礙程度的相關性,統計結果顯示客觀噪音下語句聽辨測驗的結果與主觀的自覺障礙程度間沒有呈現相關性。

本研究觀察了單側感音神經性聽損者於MHINT-T下的聽辨表現,可供後續研究或介入後之參考,並發現單側聽損者於客觀噪音下語句聽辨測驗的表現結果,與主觀問卷量表呈現的聽障程度間沒有相關性。

There are three aims of this study. The first aim was to assess the sentence recognition thresholds in quiet condition and the signal to noise ratio (SNR) at 50% sentence correct level in various noise conditions in patients with sensorineural unilateral hearing loss using Mandarin Hearing in Noise Test in Taiwan (MHINT-T). The second aim was to evaluate the self awareness of the hearing handicap level by using the subjective questionnaire Hearing Handicap Inventory for the adults (HHIA). The third aim was to analyze the correlation between the objective and subjective tests.
The control group consisted of 25 normal hearing participants with the mean age of 31.4 years old. The experimental group consisted of 16 sensorineural unilateral hearing loss participants with the age was of 38 years old.

The results of this study exhibited that the sentence recognition threshold in quiet conditions in the experimental group was 26.0 dBA with the standard deviation of 2.7 dB. The SNR at 50% correct sentence recognition in each noise conditions, such as noise front(NF), noise from the good ear(NG), and noise from the poor ear(NP) were -4.6 dB, -4.3 dB and -8.7 dB respectively. The standard deviations in each conditions (NF, NG, NP) were 1.0 dB, 1.8 dB, and 2.1dB, respectively. There were significant differences in each listening conditions between two groups. The analysis of the subjective questionnaire revealed that the mean scores in three components including total score, emotion score, and social score in HHIA were 33.3, 18.3, and 15, respectively. Four out of 16 participants expressed no handicap, nine out of 16 expressed mild to moderate handicap, and three express significant handicap, converted to 25%, 56%, and 19 %, respectively. It was found that there was no significant correlation between objective and subjective tests in Spearman correlation coefficient.

This study investigated the performance in patients with unilateral sensorineural hearing loss by MHINT-T. The findings of this study may be used as to be a reference for clinic practice. Secondly, it was observed in this study that was no significant correlation between the objective hearing test MHINT-T, and subjective questionnaire HHIA. It may be inferred that the objective test and the subjective questionnaire pertaining to hearing performance do not correlated to each other well.
