本研究針對輕度頭部受傷患者造成的不適症狀,生理症狀:頭痛、噁心、嘔吐、嗜睡、頭暈及睡眠障礙;心理症狀:憂鬱及心理疲憊,會使下肢活動下降與不負重,而導致肌肉質量減少。當肌肉質量及力量下降時,會增加衰弱、跌倒及失能的風險,故本研究目的是針對輕度頭部受傷患者,提供下肢抗阻力運動訓練與蛋白質營養支持的措施,對於主要結果變項:肌肉質量、握力、日常生活功能、生理活動功能、營養狀況及次要結果變項:憂鬱狀況、腦傷後生活品質的效用。
本研究為真實驗性研究的對照組前測與後測設計,總收案人數為55人,隨機分配兩組,介入組共28人;對照組共27人。其中五週期間介入組給予蛋白質營養補充及下肢抗阻力運動訓練,對照組則採病房常規照護。結果變項包括:肌肉質量、握力、日常生活功能、生理活動表現、營養評估、憂鬱及腦傷後生活品質。
本研究結果顯示,在生理方面,兩組下肢肌肉質量對時間在交互作用上達統計上達顯著差異(interaction effect, p=.048),並在五週期間介入組進步的平均數優於對照組。兩組上肢、全身、握力、日常生活功能及生理活動表現均隨著時間而變好;心理方面,兩組憂鬱狀況均在門診第一週時最嚴重,生活品質最差。
本次研究期間共五週,在持續給予蛋白質營養補充及下肢抗阻力運動,對於下肢肌肉質量是可以維持的,並且建議未來可運用在臥床患者,以預防下肢肌肉減少所造成的影響。
Background:
This study focused on patients who suffered from mild head injury with the post-concussion syndrome, including headache, nausea, vomiting, drowsiness, sleep disturbance, depressed mood, and fatigue. Patients who have those syndromes could become bed-ridden for a long time, which causes muscle wasting after disuse their lower limbs.
Material and Methods:
This is a prospective, randomized, experimental study with pre-test and post-test control group design. This study recruits mild head injury patients, with total sample size fifty-five. Those patients were randomized to intervention group and control group. In the intervention group, we gave higher protein support and we educated them to do lower limb resistance exercises for five weeks. In the control group, ward routine was kept.
Result:
We found that in both groups the lower limb muscle power was significantly different (p=.048) after admisson. In the intervention group, the lower limbs average muscle power and mean muscle mass is better than the control group. All of the patient's lower limb muscle power improved with time in five weeks after discharge. The most depressed mood was found in the first week after discharge. The lowest quality of life score was found at the same period of time. These results are probably related to intracranial hematoma region, intracranial anatomy and hematoma absorption time.
Discussion:
In this study, we found higher protein supplement and resistance exercises could maintain the patient’s lower muscle mass during hospitalization and home. Thus, these interventions have clinical benefits and were suggested for routine clinical care.
