臺灣博碩士論文加值系統

腎臟疾病在台灣是常見的疾病之一。當疾病邁入末期時，血液透析為主要治療方式，但是在治療過程中有許多的合併症產生，影響病人的生活品質，這對腎臟末期病患是一個很大的衝擊。本研究旨在探討末期腎臟疾病患者在接受血液透析期間（透析前與透析後）的睡眠、憂鬱及其生活品質動態變化。
研究方法採縱斷面描述性研究，立意取樣，時間自2017年6月至同年7月，對象為中部某區域教學醫院之血液透析室病患60位，資料收集為該病患連續四天的匹茲堡台灣睡眠品質量表（PSQI）、貝克憂鬱量表第二版（BDI Ⅱ）及腎臟疾病生活品質量表（KDQOLTM-36）。資料以SPSS 22.0套裝軟體進行分析。研究結果顯示，血液透析病患不論在透析前或是透析後，都具有中度的睡眠障礙（透析前10.82±4.84分，透析後10.82±5.14分）、輕至中度的憂鬱（透析前13.76±10.80，透析後16.84±11.20）及較差的生活品質（透析前237.47±60.71分，透析後230.25±63.75分）。透析第一天與第三天（透析前）的憂鬱指數顯著低於透析第二天與第四天（透析後）(p＜.001)，且透析第一天與第三天的生活品質指數顯著高於透析第二天與第四天(p＜.05 ＆ p＜.001)。
由此得知，血液透析此項醫療措施，顯著影響其憂鬱及生活品質。建議在擬定介入照護措施的時間點應該在安排非透析日進行。未來可延長監測的時間，並收集病患的睡眠數據及生理監測，另外可深入探討各等級血液透析中心的病患在睡眠、憂鬱及生活品質的差異，以增加因果推論。

Kidney disease is one of the common diseases in Taiwan. Hemodialysis is the mainstay of treatment when it comes to the end of the illness, but there are many complications that affect the patient's quality of life, which is a big shock to end-stage renal disease. This study was to investigate sleep, depression and changes in the quality of life of patients with end-stage renal disease during hemodialysis (pre- and post-dialysis).
The study was conducted on a descriptive study of longitudinal sections. Consecutive sampling was conducted from June 2017 to July of the same year. The data were collected from 60 hemodialysis patients in a teaching hospital in a certain area in a hemodialysis central. Data were collected for four consecutive days Pittsburgh Sleep Quality Scale (PSQI), Beck Depression Scale Second Edition (BDI II) and Kidney Disease Quality of Life Scale (KDQOL TM-36). SPSS 22.0 package software for data analysis. The results showed that hemodialysis patients had moderate sleep disorders (pre-dialysis 10.82 ± 4.84, post-dialysis 10.82 ± 5.14), mild to moderate depression (pre-dialysis 13.76 ± 10.80, 16.84 ± 11.20 post-dialysis) and poor quality of life (237.47 ± 60.71 pre-dialysis and 230.25 ± 63.75 post-dialysis). The index of depression on the first day and the third day of dialysis was significantly lower than that on the second and fourth day of dialysis (p <.001), and the quality of life index on the first day and the third day of dialysis was significantly higher than that on the second day of dialysis Day four (p <.05 & p <.001).
This shows that hemodialysis medical treatment, significantly affect their depression and quality of life. It is recommended that the timing of intervention interventions should be scheduled on non-dialysis days. Future monitoring can be extended and patient's sleep data and physiological monitoring can be collected. In addition, differences in sleep, depression and quality of life among patients at all hemodialysis centers can be further explored to increase causal inferences.
