People’s awareness of health has risen recently, and take care about the health care issues more rather before. The government and the private sector have also promoted smart health care actively. Using cloud technology to store individual medical records or health records on various cloud platforms, which establish a personalized health record system to Implement a personal health management and strengthen preventive health care functions. The development of a personal health record system has to integrate health records from different sources, then ensure that data is stored in a private, secure, and confidential environment. The System must offer the health records managed and used by the individual. In this study will apply blockchain technology to build Personal Health Record System. The system will integrate health records from different sources, with the use of smart contract and access control to provide the health records managed and used by the individual and keep privacy. Tamper-Proof is a feature of blockchain technology, while ensuring the security of data at the system.
