背景:失眠是現代人常見的重要問題之一。多數失眠者擔心藥物傷身或成癮,不願使用藥物,本研究希望藉由2個好取的手部穴位,提供病人方便的自我穴位按摩方法,期望提升病人使用率,進而改善病人睡眠品質及生活品質。
目的:探討穴位按摩介入措施對改善成人失眠患者睡眠品質及生活品質之成效。
方法:台灣北部某醫學中心門診及病房成人失眠患者為研究對象,方便取樣於2015年8月至2017年7月進行收案,隨機分派完成收案數為實驗組51人及對照組52人。實驗組教導自我連續四週穴位按摩措施及給予睡眠衛生行為衛教,對照組僅進行睡眠衛生行為衛教。測量工具為匹茲堡睡眠品質量表及台灣簡明版生活品質問卷進行評估。所有個案於收案當天進行前測,第二週、第四週各執行一次後測。統計方法以獨立t-test、paired t-test以及複迴歸分析進行介入成效檢定。
結果:根據paired t-test分析結果顯示,實驗組在接受穴位按摩介入措施後,其睡眠品質結果之睡眠品質總分(p < .001)、自覺睡眠品質(p = .006)、入睡時間(p < .001)、睡眠時間(p = .015)及睡眠效率(p = .038)等面向有顯著改善,其生活品質結果之生活品質總分(p = .028)及心理範疇(p = .004)有顯著改善。根據獨立t-test及複迴歸分析結果顯示,實驗組在接受穴位按摩介入措施後,睡眠品質之睡眠品質總分及入睡時間較對照組改善,生活品質之生活品質總分與生理範疇較對照組改善。實驗組與對照組皆給予睡眠衛生行為衛教措施後,其睡眠衛生行為皆無顯著改善。
結論/實務應用:本研究結果支持穴位按摩介入措施可改善睡眠品質及生活品質,此結果提供非藥物改善失眠問題之實證依據,建議未來可納入護理常規之參考。

Background: Insomnia is one of the most common problems in modern life. Most of the insomniacs are concerned about drug-induced injury or addiction and are reluctant to use drugs. In this study, we try to provide patients with a convenient self-acupressure by means of 2 well - acupressure points, which may improve the patient's compliance as well as sleeping quality and life quality.
Purpose: Firstly, we discussed whether acupressure might improve sleep quality in adult patients with insomnia. Second, we try to make sure that acupressure may also improve life quality in adult patients with insomnia.
Methods: Patients who was diagnosed as insomnia from outpatient department and ward in a medical center in northern Taiwan were involved from August 2015 to July 2017. All included cases were randomized in the experimental group and control group, 51 patients and 52 patients respectively. The experimental group was taught sleep hygiene behavior education and acupressure. The control group was only taught for health education. We used the Pittsburgh Sleep Quality Scale and the Concise Life Quality Questionnaire for Taiwan to measurement the sleeping quality of these patients. All cases were measured on the day, 2 weeks after the day, and 4 weeks after the day. We also used Statistical Methods Paired t-test for interventional efficacy test.
Results: According to paired t-test analysis, the experimental group did significantly improve the quality of sleep scores (p < .001), sleep quality (p = .006), fall asleep time(p < .001)and sleep time after receiving acupressure intervention (p = .015) and sleep efficiency (p = .038), as well as the experimental group after receiving acupuncture point massage intervention, the total quality of life (p = .028) and psychological (p = .004) is significantly improved. According to independent t-test and complex regression analysis results show that the experimental group after receiving acupuncture point massage intervention measures, sleep quality of sleep time and sleep quality score than the control group to improve the quality of life of the physical category and quality of life than the control group improve. The experimental group and the control group were given sleep hygiene behavior of health education measures; the sleep hygiene behavior did not significantly improve, but also confirmed that the experimental group of health effects of sleep hygiene does not affect the effectiveness of acupressure intervention.
Conclusions/Implications for Practice: This study shows that acupressure does improve sleep quality and quality of life. This result provides a non-drug basis for the improvement of insomnia. It is suggested that future reference should be made to nursing practice.
