目的：人口老化與獨居老人增加此兩趨勢，將雙重提高老人的醫療利用，而健康狀態亦是影響醫療利用主要的原因。因此本研究探討台灣老年人不同居住安排、健康狀態對醫療利用之相關性，並使用Andersen第一階段醫療服務利用行為模式作為控制因素，深入了解老年人醫療利用情形。
方法：本研究為回顧性研究，利用衛生福利部衛生福利資料科學中心所提供之衛生福利資料檔進行次級資料分析。以2013年之國民健康訪問調查檔為分析檔案，採用65歲以上之老年人為研究分析對象，將其與全民健保處方及治療明細檔_門急診、全民健保處方及治療明細檔_住院等資料互相串聯進行分析，獲更詳細之醫療利用情形。以描述性統計、獨立樣本t檢定、單因子變異數、線性複迴歸與負二項式迴歸分析等方法分析，為探討影響老年人醫療利用之相關因素，將每項醫療利用建立四個迴歸模式，加入傾向因素、使能因素、需求因素作為控制變項。而門診費用、急診費用與住院費用等依變項，皆為右偏分布而非常態分布，因此將其以取對數(log)的方式進行校正。
結果：在控制傾向因素、使能因素、需求因素後，獨居者比非獨居者有較多的醫療利用情形，但僅急診使用次數達顯著水準。在健康狀態與醫療利用方面，自評健康是影響醫療利用的顯著因素；老年慢性疾病狀況顯著影響門診總費用、門診次數及住院次數；行動能力顯著影響門診總費用及住院天數；日常生活活動評估顯著影響住院費用及住院次數、工具性日常生活活動評估顯著影響門診總費用、急診總費用、門診次數、急診次數及住院次數。
結論：居住安排為獨居與健康狀態較差之老年人，會有較多醫療利用，若能重視老年人獨居情形與其健康狀態，根據不同老年人給予不同且正確的醫療服務，使獨居老人能適時使用所需之醫療資源，才能有更好的醫療照護品質。

Objective: This study aimed to investigate the relationships of living arrangements and health status with healthcare utilization among the elderly Taiwanese population. Control factors were established using Andersen’s Behavioral Model to gain an in-depth understanding of healthcare utilization by the elderly.
Method: In this retrospective study, secondary data analysis was performed on data provided by the Ministry of Health and Welfare. The 2013 National Health Interview Survey Data was used for analysis, and the elderly aged 65 and above were selected as study subjects. The independent t-test, one-way ANOVA, linear regression and negative binominal regression were used to analyze the relevant factors influencing healthcare utilization the elderly. Four regression models were constructed for each healthcare service and predisposing, enabling, and need factors were included as control variables.
Result: After controlling the predisposing, enabling, and need factors, it was found that the elderly who lived alone had higher healthcare utilization than those who did not live alone; however, a significant difference was only observed in the number of emergency department visits. With regard to the relationship between health status and healthcare utilization, self-rated health was a significant factor that affected healthcare utilization; chronic diseases and conditions significantly affected total outpatient expenditures, number of outpatient visits, and number of hospitalizations; mobility significantly affected total outpatient expenditures and length of hospitalization; ADL significantly affected hospitalization expenditures and number of hospitalizations; IADL significantly affected total outpatient expenditures, total emergency care expenditures, number of outpatient visits, number of emergency department visits, and number of hospitalizations.
Conclusion: This study demonstrated that the elderly who live alone and poor health status have higher healthcare utilization.
