芳香按摩對高齡者乾眼症改善成效之探討

摘 要
本研究主要目的在探討高齡者乾眼症經過8週芳香按摩之後,在淚液分泌、自律神經功能調節及睡眠相關指數之改善成效作探討。方法:本研究是以44名高齡者為受試對象。實驗分成三組:A組為對照組,參與受測者為16位(平均年齡68.25±2.01歲), B組為按摩組13人(平均年齡69.08±1.60歲),施予眼部周圍佐以基礎油按摩,C組為精油按摩組15人(平均年齡66.27±2.32歲),施予眼部周圍佐以精油按摩,按摩時間5-6分鐘,按摩頻率每週2次,如此1個循環,總共進行8個循環,時間維持2個月。實驗皆於按摩介入前、介入中第4週、介入後第8週,對受試者進行淚液分泌試紙測試(Schirmer’s Test)、淚膜破裂時間測試(Tear break-up time)、眼表分析儀(OSA)、心率變異分析儀(HRV)以及腦波儀(EEG)作檢測。精油選擇快樂鼠尾草及紫檀木精油,調配濃度經試管外(in vitro)培養人類視網膜色素上皮細胞(human retinal pigment epithelial cell; ARPE-19 cell)毒性實驗後得到,快樂鼠尾草及紫檀木精油稀釋在基礎油的安全濃度為0.8%。統計工具使用單因數變異分析,組內進行成對樣本t檢定,顯著性設p<.05。研究結果:各組的淚液試紙測試及淚膜破裂時間是無相異的。心率變異分析結果顯示:心跳速率於B組及C組介入第4週、第8週是降低的,低頻功率則在C組介入第8週後,按摩後比按摩前是增加的,高頻功率及正規化高頻功率比在C組介入第4週及第8週後是增加的。眼表分析儀部分則發現在脂質層厚度、眨眼品質、淚河高度及非侵入性破裂時間則發現B組在介入第4週後可看出前兩項有明顯的差異,而C組則在介入第4週及第8週後此四項生理參數均明顯的改善。腦波儀的部分則發現B組在介入第4週後在壓力、放鬆、專注力及疲勞指數明顯改變,C組則在介入第四週睡眠指數及介入第8週後壓力、放鬆、睡眠指數均有明顯的改善。綜上所述,結論:透過芳香按摩技法後對自律神經之副交感神經活性可以獲得提升,並有助於緩解乾眼症的不適症狀,且可達到舒緩心理壓力使身體處於相對放鬆狀態,亦同時改善睡眠品質。

The Effect of Aroma Massage on the Dry Eye Syndrome in the Elderly

Abstract
OBJECTIVE This study is to investigate the improvement of dry eye and emotion-related index in elderly after 8 weeks of aromatic eye massage. METHOD The ethics committee of human behavior research of Yuanpei Medical Science and Technology University approved all study procedures. 44 elderly people (over 50 years old) were selected and greater than 13 in Ocular Surface Disease Index (OSDI), and excluded elderly people with very severe dry eye caused by LASIK surgery. The subjects was divided into three groups. The first group: no massage will be performed for 16 people, the second group: 13 people will be performed with base oil massage and the third group: 15 people will be performed with essential oil massage around the eyes. The clary sage essential oil and rosewood essential oil were used through the eye around the aroma massage for 5 to 6 minutes, the frequency of massage twice a week, such a cycle, a total of 8 cycles. The Schirmer’s test, tear break-up time, ocular surface analyzer (OSA), heart rate variability (HRV) and electroencephalograph (EEG) were measured before the experiment, after 4 weeks’ massage, and after 8 weeks’ massage. The position of the aroma eye massage is applied on the location of the parasympathetic nerves. The brain wave in EEG were proceeded and then performed stress, concentration, sleep, relaxation and fatigue.Statistic tools used single-factor variation analysis, and paired t tests are performed within the group and the significance is set to p<.05. RESULTS In our preliminary toxicity test of human retinal pigment epithelial cell (ARPE-19 cell) in vitro, we found that the safety dosage of the clarysage and rosewood essential oil were 0.8% in the base oil after percutaneous absorption of the eyelids. The noninvasive break up time, lipid layer, eyeblink quality, normalized high frequency (HF%) in HRV analysis and sleep index in EEG showed that the experimental group and the control group are significantly different. The results can provide us a reference for aromatherapy in the future.
