背景：髖關節骨折發生率高居全世界前十名，在臺灣的長者亦相當常見，急性後期整合照護(Post-acute Care，PAC)在急性後期之治療黃金期內立即給予積極性之整合性照護，使其恢復功能，將可減少後續再住院醫療費用、減輕家庭及社會照顧之負擔。
目的：本研究目的在探討一、長者髖關節骨折術後接受急性後期整合照護之功能狀況恢復的成效。二、長者與家屬接受PAC歷程的經驗感受。
方法：研究採量性與質性混合研究的方法，以立意取樣(purposive sampling)，量性方面採病歷追溯法，進行追蹤收集159位年滿65歲或65歲以上長者的基本資料、滿意度及於執行前後日常生活活動功能(Activity of Daily Living，以下簡稱ADL)-巴氏量表 (Barthel Index)、疼痛程度-疼痛指數量表(Numerical Rating Scale，以下簡稱NRS)、髖關節的殘障程度-髖關節骨折評估表(Harris Hip Score，以下簡稱 HHS)分數的差異以作為分析評估其功能狀況恢復成效的探討，統計使用SPSS 25.0套裝軟體，檢驗成對樣本t檢定(Paired Samples t-test) 、皮爾森相關係數(Pearson correlation coefficient )、卡方檢定(Chi-square Test) 、雪費精確檢定(Fisher's exact test) 、曼惠二氏U檢定法(Mann-Whitney U Test )、克-瓦二氏單因子等級變異數分析( Kruskal Wallis Test)，以進行統計分析。質性方面則於研究分析過程中，透過立意取樣，以訪談髖關節骨折術後接受PAC的長者之及家屬，以資料分析飽和為收案原則。結果共訪談9位個案及9位家屬，，採用內容分析的方式，同時運用老年憂鬱量表(Geriatric Depression Scale-5 Item，以下簡稱GDS-5)，以探討PAC執行前後過程的經驗感受。
結果：本研究結果顯示量性部分長者髖關節骨折術後接受PAC的ADL分數從46.98±8.25增加到62.74±12.49（p = .000 ***），ADL得分有明顯改善。NRS分數從3.92±1.41降低至2.58±1.27（p = .000 ***），顯示出顯著改善。 HHS分數從35.11±12.75增加到47.92±13.71（p = .000 ***），也顯示出明顯的改善。 另外，長者髖關節骨折術後接受PAC治療的功能恢復較好，抑鬱程度較低，滿意度較高。ADL、NRS、HHS、憂鬱程度均有相關性影響。質性結果則歸納出5個主題，包括:一、無力感; 二、自我照顧能力喪失三、對未來無望四、害怕變成負擔或被放棄五、重獲自我的統整。
結論：本研究結果證實PAC具有重要成效，且多於或等於10天為最適切復健天數。而於質性研究結果則發現個案及家屬仍會面臨許多的生理、心理困擾。本結果可作為國家規劃健康政策參考，同時提供專業人員設計PAC介入參考，使個案盡早恢復活動功能，提升生活品質，減輕家庭及社會照顧之負擔。

Background: The incidence of disability due to hip fractures ranks among the top ten in the world. It is also prevalent in the elderly in Taiwan. The purpose of this study is to explore the effectiveness of post-acute care (PAC) and recovery of functional status in elderly patients with hip fracture surgery under the current Taiwan Diagnosis-related groups (DRG) payment system.
Objective: First of all, the purpose of this study was to explore the effectiveness of post-acute care (PAC) function recovery after hip fracture in the elderly. Secondly, this study will also explore the elderly and their family members' experiences while under the PAC.
Methods: This mixed-methods study involves both quantitative and qualitative research design. First, quantitative research used a retrospective chart review method. It was used to collect the data of 159 seniors aged 65 and over elderly about before and after PAC. The differences between the before and after receiving PAC were collected about the Barthel Index (ADL), the pain index scale (NRS), and the hip fracture assessment table (HHS) by using purposive sampling. The Statistics SPSS 25.0 package software was used to test the Paired Samples t-test, Pearson correlation coefficient, Chi-square Test, Fisher's exact test, Mann-Whitney U Test (Mann-Whitney U Test) and Kruskal Wallis Test (Kruskal Wallis Test) for statistical analysis. The qualitative aspect was followed by sampling during the analysis of the study. It is expected that 9 family members and 9 elders who received PAC after hip fracture surgery will be interviewed until the data is saturated. And use the method of content analysis, combined with the Geriatric Depression Scale-5 Item (hereinafter referred to as GDS-5), to explore the experience of PAC before and after the process of experience.
Results: The results showed that the ADL of the elderly with hip fracture before and after receiving PAC increased from 46.98 ± 8.25 to 62.74 ± 12.49 (p = .000 ***), the score of ADL showed significant improvement. The score of NRS decreased from 3.92±1.41 to 2.58±1.27 (p = .000 ***), showing significant improvement. The score of HHS increased from 35.11±12.75, to 47.92±13.71 (p = .000 ***), also showing significant improvement. And the results showed that the elderly who received PAC after hip fracture surgery had a better functional recovery, lower depression, and better satisfaction. ADL, NRS, HHS, and depression degree all have a correlation effect. The qualitative results summarized 5 themes, including 1. Powerlessness; 2. Loss of self-care ability 3. Hopelessness for the future 4. Fear of becoming a burden or being abandoned 5. Regaining self-integration.
Conclusion: The results of this study confirm that PAC has important effects, and more than or equal to 10 days is the most appropriate rehabilitation days. The qualitative research results found that the case and family members still face many physical and psychological processes. This result can be used as a reference for national planning health policies and cross-disciplinary professional design cases and family PAC intervention plans, in order to restore activity functions as soon as possible, improve the quality of life, and reduce the burden of family and social care.
