本研究目的在於建構一份具消防工作適用性且信效度佳之消防人員創傷經驗後反應量表,研究流程分為三個階段。首先,研究者招募三位在消防工作中具有創傷經驗的資深消防人員,透過實際訪談了解其在經歷創傷經驗後所呈現的生理及心理壓力反應,並依消防工作常見創傷事件編製本量表的第一部分創傷事件;接著,研究者採兩階段翻譯方式將PTSD Checklist for DSM-5 (PCL-5)量表題項進行中文化,並邀請兩位專業人員確認題目內容,以確保本量表之內容效度,並加入研究者自編織題目後,形成預試量表第二部分創傷經驗後反應共30題;預試量表共招募142位消防人員參與施測,經過項目分析與探索性因素分析後,形成消防人員創傷經驗後反應正式量表共23題,包含四個分量表分別為「生理警覺反應」、「心理疏離」、「死亡焦慮」及「無望感」,採李克特5點量表,依困擾程度分為0至4分,受試者的分數越高,表示創傷經驗後反應對其造成的困擾程度越強烈。
最後,正式量表共招募536位國內消防分隊之分隊長、小隊長及隊員進行施測,並同時施以「華人心理健康量表」(CHQ-12)作為效標工具。正式量表全量表之內部一致性信度為.97,各分量表則介於.86至.95;兩週後針對91位重測樣本進行重測,全量表重測信度為.64;正式量表經驗證性因素分析,各項適配指標均顯示本量表具有良好之建構效度(GFI=.89, RMSEA=.06, CFI=.96, NFI=.94, RFI=.93, IFI=.96, TLI=.95),而與CHQ-12之效標關聯效度為.70;此外,本研究也發現越資深的消防人員有越高的創傷經驗後反應程度,最後研究者針對研究結果提出結論及建議。

This study aims to develop a reaction scale of traumatic experiences for the firefighter. We conducted three phases during the process of scale development. In the first phase, individual depth interviews were conducted to collect the data from three senior firefighters with traumatic experiences during work. A pilot scale was developed based on these interview data and the PTSD Checklist for DSM-5 (PCL-5) scale. In the second phase, after assessing content validity from two experts, a 30-item original scale was conducted as a pretest of 142 participants, and then the data was analyzed and selected under item analysis and exploratory factor analysis methods to establish a formal scale. The Firefighter’s Reaction Scale of Traumatic Experiences (RSTE) contains 23 items rated on a 5-point scale and consists of four subscales: trauma-related physiological reactivity, psychological alienation, death anxiety and hopelessness. Higher scores people in the scale show strong reactions of traumatic experiences. In the third phase, the participants which were 536 firefighters from the fire department in Taiwan took the formal scale (RSTE) and then the Chinese version of General Health Questionnaire (CHQ-12) as a validation tool to obtain the criterion-related validity. Reliability analysis indicated that the internal consistency of the four subscales was between .86 and .95, and the internal consistency of the whole scale was .97. Two-week test-retest reliability of 91 participants was .64. The comparison of background variable indicated that the scores for senior workers were significantly higher than the scores for junior workers. Confirmatory factor analysis showed a good model fit (GFI=.89, RMSEA=.06, CFI=.96, NFI=.94, RFI=.93, IFI=.96, TLI=.95), which supported good construct validity. The scale also demonstrated high criterion-related validity with physical and mental health as criteria (r = .70). Further, the discussion and implication based on the results are provided.
