Abstract
Emergency medical care service is one of the jobs of a fireman, its aim is to minimize the condition of getting worse from patients before arriving a hospital. In recent years, the demand from people for such emergency medical care service is being increased. The statistics, from National Fire Agency, Ministry of the Interior, suggests that the case of nationwide emergency medical care service has been surged by 10.84% from 2011 to 2020, in which among them, according to the Fire Bureau from Tainan City Government, received cases were more than 92,000 in 2020, meaning over 250 cases in average on a daily basis.
With the information mentioned as above knowing that how vast of such emergency medical service requires, in respond to the increasing demand and in actually improvement the survival chance to OHCA patients the firemen are trained with rigid medical courses and collaborated with the medical care networks from all hospitals to exactly reinforce its procedure between two sides. Furthermore, the Fire Bureau from Tainan City Government sets up a specific medical securing squad, organizing a training course for senior emergency medical technicians and equipping ambulances with AED and LMA, which is for the purpose of getting more improvements to the procedure of emergency medical care in terms of training and kits. The situations, however, of medical care service being abused by people in many more cases are not rare to be seen primarily because of the reason that people do not fully understand the mechanism of using services properly. In addition, they also take advantage of ambulance’s accessibility and free of charge, which causes the entire abusing cases even worse.
In every city so far, still the emergency medical care services are the busiest and the most physical- consuming work that firemen take, and even quite vulnerable in relation to manpower allocation. To reform such circumstances and to be able to truly take good care of those people in need, there are eleven cities across Taiwan established a charging system for ambulance using. Therefore, the Fire Bureau from Tainan City Government instructed “the charging implementation of ambulance in Tainan” with regards to some people constantly called the ambulance services in abuse, from which clearly pointed out the expense if some violations occur. Yet, it is worth to do more researches on how to effectively reduce resources to be abused and wasted, and teach people a proper mechanism of using as well as to meet social expectation of exactly boosting the entire quality of medical care service. This thesis aims to analyze the abusing and allocating situations of the resources of emergency medical care services, and how to make arrangement and level management with efficiency in integration current policies, regulations and other measures, discussing the questions regarding the measures at present towards abusing ambulance service and the level of effectiveness of improvements by providing relative suggestions. Following questions for exploration are listed based mainly on the level management of Taiwan emergency medical care system, the root-cause of abusing resources and the potential problems to the policy of ambulance charging and how to actually response: 1. To acknowledge of how Tainan City Government arrangement system of emergency medical care service and its level management would be?
2. Exploring the causing of emergency medical care service in abuse in Tainan City and how to improve?
3. The policy of charging if ambulance uses in abuse in every city including Tainan City. 4. How to react the situation of ambulance and manpower with insufficiency? 5. Coping with the possible measures in accordance with the researching.
A comprehensive view above, to modify the abusing problems to the resources of Taiwan emergency medical care, this thesis refers to all the relevant literatures, documents and figures to explore what the current dilemma would be in order to actually find out how to cope with. Thus, maximizing the function of medical care service and exercising its resource for those in need.
