背景:老年人隨年齡增長,需面臨喪偶及老友離世等因素,導致社交互動減少,加上身體面臨功能衰退而入住機構,面臨重大的生活改變,心理層面容易導致幸福感降低及低自尊。
目的:旨在探討介入藝術治療,對長期照顧機構老年住民之社交互動、自尊及幸福感的成效。
方法:本研究採類實驗兩組前、後測研究設計,以臺灣北部地區長期照顧機構(養護型)的老年住民為研究對象進行收案。實驗組進行每週一次,每次90分鐘,為期八週之藝術治療,對照組則維持原本生活作息。於第一週介入前及第八週介入結束後以社交互動量表、羅斯伯自尊量表及老人幸福感量表進行測量。統計資料建檔及分析以IBM SPSS Statistics 22.0,並以描述性統計、卡方檢定、獨立t檢定、配對t檢定、單因子共變數分析及詹森內曼法等統計方法進行資料分析。
結果:實驗組有34位與對照組有35位,共69位長期照顧機構老年住民完成研究。實驗組於八週藝術治療後,社會支持性活動評量表從67.50分(SD=13.39)提升至76.8分(SD=11.38)、羅斯伯自尊量表從27.18分(SD=5.23)降低至20.09分(SD=5.42)、老人幸福感量表自23.79分(SD=6.33)提升至33.82分(SD=7.40),於後測均達到統計顯著提升(p<.05);對照組則無統計顯著差異(p>.05)。經單因子共變數分析顯示,社交互動不符合組內迴歸係數同質性,自尊及幸福感均符合,經單因子共變分析及詹森內曼法顯示,於介入後能有效提升社交互動、自尊及幸福感(p<.05)。
結論/實務應用:本研究結果證實八週藝術治療能有效提升長期照顧機構住民社交互動、自尊及幸福感,建議長期照顧機構能將藝術治療融入於日常活動設計中,藉以豐富老年住民晚年生活。

Background: When aging, older people face widowhood, and the death of old friends with increasing age, leads to reduced social interaction. In addition to declining physical function, they are admitted to institutions, and significant life changes lead to lower psychological well-being and low self-esteem.
Aim: To investigate the effects of art therapy on social interaction, self-esteem, and well-being for elderly residents in long-term care institutions.
Method: A quasi-experimental study was conducted on 69 residents of long-term care institutions in northern Taiwan. Thirty-four participants in the experimental group received eight weeks of art therapy, and 35 participants in the control group maintained their original routine. Both groups were pre-tested and post-tested with the Socially Supportive Activity Inventory, Rosenberg Self-Esteem Scale, and Well-Being Scale for elders in the first and eighth weeks. SPSS 22.0 was employed for data analysis. The chi-square test and the independent sample t-test were used to examine the homogeneity of the experimental and control groups, and one-way analysis of covariance (ANCOVA) was used to examine the effectiveness of art therapy. Johnson-Neyman was used instead of the regression coefficients were heterogeneity within the group.
Results: According to the results of the chi-square test and independent sample t-test show that there are no significant differences (p>.05) between the experimental group and the control group in their demographics (gender, age, length of residence, religion, education level, marital status, economic status, and the function of daily living activities) and disease characteristics (depressive symptoms, cognitive function, and chronic illness). The mean score of the Socially Supportive Activity Inventory in the experimental group increased from 67.50(SD=13.39) to 76.8(SD=11.38). Rosenberg Self-Esteem Scale means score dropped from 27.18 (SD=5.23) to 20.09 (SD=5.42), and Well-Being Scale for Elders mean score improved from 23.79 (SD=6.33) to 33.82 (SD=7.40). There was a significantly increased in the post-test scores (p<.05); the control group was no statistically significant difference (p>.05).
Conclusions/Implications for Practice: The results of this study confirmed that eight weeks of art therapy could effectively improve the social interaction, self-esteem, and well-being of elderly residents in long-term care institutions. It is hoped that the results will provide a reference for the multiple care models of nursing staff in long-term care institutions to enhance and improve the later years of residents.
