臺灣博碩士論文加值系統

本研究以中央制定的補助老人裝置假牙政策交各縣市政府執行出現不同的實施內容，加上半數的縣市政府擴大實施對象，衍生出一個制度兩個方向及多個差異內容的情形，故本文主要探討全國無牙老人之公平性與納入健保制度的合理性。透過整合文獻與各級政府的公開資訊，從各縣市政府的財政收支與公共債務、無牙老人的困窘狀況與對配置假牙的期待、補助老人裝置假牙政策在各地方政府之間的實施差異與嚴重的人口老化加重未來勞動人口的負擔等四個面向，以文獻分析法、資料分析法與分組數字法等質性研究方法進行縱向與橫向分析，並針對已配戴活動式假牙的老年人及牙醫師等二類人員進行深度訪談，得到其內在真實觀點做為佐證論點之實務依據。補助老人裝置假牙政策納入全民健保制度對於固有的執行模式將帶來衝擊，在考量實際面與改進面之間的得失關係時，提供未來權衡納入全民健保制度的合理性評估之參考資料。本研究建議將其收歸中央政府統一執行，納入全民健康保險制度，以實現對滿口無牙或嚴重缺牙的全國65歲以上老年人及55歲以上原住民之公平性。

In this study, the policy of denture subsidies for the elderly formulated by the central government and implemented by the county and city governments has given rise to different implementation contents. In addition, 50% of the counties and cities have extended their implementation objectives, leading to a system having two directions and multiple differences in contents. In order to explore the fairness of the Policy for the toothless elderly nationwide and the reasonability of inclusion in the National Health Insurance (NHI) system, the literature and public information at all levels of government were integrated. They include four aspects: The fiscal balance and public debt of respective county and city governments; the dire circumstances of toothless elderly and their expectations towards getting dentures; the differences in the policy of denture subsidies for the elderly implemented by the local governments; and severe population aging that only burdens future labor force population. The document analysis, data analysis method, nominal group technique and other qualitative research methods were adopted to carry out longitudinal and horizontal analysis. In addition, targeting two categories of individuals, namely, the elderly already wearing movable dentures and dentists, in-depth interviews were conducted. The actual inner viewpoints served as the practical basis of the supporting arguments. The inclusion of the policy of denture subsidies for the elderly in the NHI system has impacted the existing mode of implementation. Considering the gain and loss relationship between the practical side and the improvement side, the reference data for evaluating the reasonability of its future inclusion into the NHI system after weighing the pros and cons were provided. It is suggested in this study that the inclusion in the NHI system be reclaimed and uniformly implemented by the central government, thereby realizing the fairness for the elderly aged above 65 and Indigenous peoples aged above 55 who are toothless or have multiple missing teeth.
