研究背景:科技時代來臨與疫情時期,智慧醫院、數位醫護成為重要的趨勢。傳統護理作業,護理師耗費許多時間於手寫紙本護理過程紀錄、紙本記帳、人工彙整與書寫交班單,易有遺漏紀錄書寫、記帳、交班事項之情況發生。醫護作業精密,高複雜度且具高風險特性,更須藉由科技資訊化,提昇醫療照護效能。
目的:於個案醫院規劃導入護理資訊系統,簡化流程,提升使用者滿意度與工作效益。探討記帳系統、交班系統、護理資訊系統整體的成功推展因素,以及成功因素對於這些系統「使用滿意度」的影響,同時探討使用滿意度對工作績效的影響,以及這些系統的效益。
研究方法:跨部門合作,以使用護理資訊系統3個月以上之護理師進行問卷調查,收集評價資料。首先,以SPSS 20.0統計軟體進行敘述性統計分析、單因子變異數分析,以內容分析法進行分析開放性問題回答資料;接著以結構方程式的統計工具Smart PLS 3.0,針對結構模型中路徑係數與模型解釋力檢定,進行架構驗證。
結果:經研究驗證顯示主觀規範、系統品質等成功因素,正向顯著影響記帳系統之使用者滿意度與護理師的工作績效;科技特性正向且顯著影響交班系統之任務-科技適配度,資訊品質、服務品質正向且顯著影響交班系統之使用者滿意度,任務-科技適配度、使用者滿意度正向且顯著影響工作績效。最後,驗證顯示主觀規範、系統品質、資訊品質、服務品質和任務特性等成功因素,對護理資訊系統整體使用者滿意度的影響能力高達83.1%;使用者滿意度對使用者效益的整體影響能力高達78.0%,此研究整體顯示模式解釋程度良好。
結論:個案醫院藉由資訊種子、示範單位成功導入護理資訊系統經驗,增設無線網路,分階段逐步擴散至全院成功導入護理資訊系統。顯示醫院主管對護理資訊系統上線支持度之重要,系統除重視科技面資訊因子品質,資訊工程師服務品質外,更須考量護理作業之任務特性,促進護理資訊系統更符合臨床照護病人的需求,增進工作效率,提升護理師使用滿意度及效益。

Background: With the advent of the era of technology and the COVID-19 pandemic, smart hospitals and digital healthcare have become an important development trend. In traditional nursing practice, nurses spend a lot of time on manual tasks such as recording nursing history, completing accounting information, compilation, and shift handovers, which are prone to human errors and omissions. Medical operations are precise, complicated, and risky, thereby requiring the implementation of information technology to improve efficiency.
Objective: Import the nursing information system (NIS) into the case hospital to simplify the process, improve user satisfaction and increase work efficiency.
This study also explores factors that could promote the successful implementation of the accounting, the shift, and the nursing information systems, as well as investigating the effects of these factors on “user satisfaction.” Meanwhile, the impact of user satisfaction on work performance was also discussed together with the effectiveness of the aforementioned systems.
Methods: Through cross-departmental collaboration, a questionnaire survey was conducted among nurses who had used the Nursing Information System for more than three months. The SPSS 20.0 statistical software was adopted for data analysis, such as descriptive statistical and one-way ANOVA. The content analysis was used to investigate responses to open-ended questions. Subsequently, structural equation model (SEM) was used to explore the research structure model and model’s path coefficients by Smart PLS 3.0.
Results: Success factors such as subjective norms and system quality demonstrated significantly positive impacts on the user satisfaction of the accounting system and the work performance of nurses. Secondly, technology characteristics exhibited a significantly positive effect on the task-technology fit of the shift system, while both information quality and service quality displayed significantly positive impacts on the user satisfaction of the shift system. Finally, in the NIS, task-technology fit and user satisfaction demonstrated significantly positive effects on work performance. The model results indicated that the user satisfaction on NIS can explain as high as 83.1% of the work performance variation. The variation of user efficiency explained by user satisfaction was as high as 78.0%, which showed the good fit of models.
Conclusion: The case hospital successfully introduced the nursing information system through information seed personnel and demonstration units. The case hospital added a wireless network, which was gradually spread in stages, and the nursing information system was successfully introduced to the whole hospital. The experience of the case hospital shows the importance of the hospital supervisor's support for the implementation of the nursing information system. The system not only pays attention to the quality of information factors in technology and the service quality of information engineers, but also needs to consider the task characteristics of nursing operations, so as to promote the nursing information system to be more suitable for clinical care of patients It can improve work efficiency and improve the satisfaction and efficiency of nurses.
