手術室護理人員因工作需要長時間維持同一姿勢、搬重物、重複傳遞器械或不當的姿勢,導致肌肉骨骼不適;也因工作時間長及工作壓力大,常造成職業疲勞的問題。肌肉骨骼不適及職業疲勞經過長時間的累積容易造成手術室護理人員身、心、社會的負荷,進而影響工作表現與身心健康。國內外文獻鮮少以手術室護理人員為研究對象,同時探討其肌肉骨骼不適及職業疲勞情形,因此本研究的目的為探討手術室護理人員肌肉骨骼不適、職業疲勞及其影響因素。
本研究採量性橫斷式設計,自2022年05月至06月期間,以立意取樣選取台灣北、中、南各地區區域級教學醫院(含)以上的手術室護理人員為研究對象,以基本資料表、中文版北歐肌肉骨骼問卷、以及中文版哥本哈根疲勞量表進行資料收集,有效問卷共117份。以SPSS 22.0進行描述性分析、獨立樣本t檢定、單因子變異數分析、皮爾森積差相關分析、羅吉斯迴歸分析及全部進入複迴歸分析。
研究結果顯示手術室護理人員發生肌肉骨骼不適比例最高的前三個區域為下背或腰部(76.1%)、頸部(70.9%)及肩部(70.9%),均為中等疼痛。女性、在醫學中心手術室工作、中及高度工作負荷、不常運動、年齡愈大、手術室年資愈久、BMI指數愈高及每天平均睡眠時數較多者,其肌肉骨骼不適較嚴重。醫院級別、運動習慣及工作負荷程度為肌肉骨骼不適的預測因子。職業疲勞方面,手術室護理人員有輕度的整體疲勞感,但個人疲勞構面屬於中度疲勞最為嚴重。高工作負荷、每週平均跟刀時數愈長及BMI指數愈高的手術室護理人員,其職業疲勞愈嚴重。工作負荷程度為職業疲勞的預測因子,可解釋總變異量15.5%(F = 5.24, p < .001)。本研究結果期望能提供醫院或手術室單位早期發現手術室護理人員肌肉骨骼不適或職業疲勞的問題,並作為制訂改善措施之參考。
Operating room nurses often experience musculoskeletal discomfort from maintaining the same posture for an extensive period, lifting heavy objects, and repeatedly delivering surgical instruments in an inappropriate posture. Also, long working hours and high work pressure often cause job burnout. Long-term musculoskeletal discomfort and job burnout easily impose burden on the physical, psychological, and social well-being of operating room nurses, which in turn affect their work performance as well as their physical and mental health. Studies in Taiwan and other countries have rarely explored the musculoskeletal discomfort and job burnout of operating room nurses simultaneously. Therefore, the purpose of this study was to investigate the musculoskeletal discomfort, job burnout and their related factors in operating room nurses.
This quantitative study was a cross-sectional design. By purposive sampling, the operating room nurses was recruited with the medical centers or regional teaching hospitals in north, central and south Taiwan from May 2022 to June 2022. The instruments including the basic information form, Nordic Musculoskeletal Questionnaire-Chinese version, and Copenhagen Burnout Inventory-Chinese version, were used to collect data. The total valid questionnaires were 117. The obtained data were analyzed with SPSS 22.0 by using descriptive analysis, independent sample t test, one-way ANOVA, pearson’s correlation analysis, logistic regression, and all enter regression analysis.
The results showed that the three body areas with the highest proportion of musculoskeletal discomfort among operating room nurses were the lower back or waist (76.1%), neck (70.9%), and shoulders (70.9%), all with moderate pain. Operating room nurses who were female, worked in operating rooms of medical center, had moderate to high workloads, or did not exercise frequently tended to have more serious musculoskeletal discomfort, with the discomfort increasing with increasing age, work experience in an operating room setting, BMI, and sleep hours per day. Hospital level, exercise habits, and workload were predictors of musculoskeletal discomfort. Operating room nurses showed mild overall job burnout, but experienced moderate personal burnout that was the most serious one in all dimensions. Operating room nurses with high workloads, longer average surgical time per week, and higher BMI had more severe job burnout. Workload was a predictor of job burnout, and the total explicable variance was 15.5% (F = 5.24, p < .001). These findings were expected to provide hospitals or operating units with allowing early detection of musculoskeletal discomfort or job burnout for operating room nurses and serving as references for developing improved measures.
