本研究旨在探討「身體症狀疾患」之有效治療策略。本研究兼採 量化研究與質性研究。研究對象為十名診斷為身體症狀疾患之個案, 並依個案意願將其分為二組,一組接受純粹藥物治療,另一組接受合 併心理教育介入方案之治療方式。心理教育介入方案為一全面性的治 療概念,涵蓋症狀評估、認知行為治療、心理教育介入、家族治療、 與生態系理解、多面向分析。其中症狀評估部分藉由臨床整體評估嚴 重度指數、社會與職業功能評估量表、身心症狀問卷三項量化指標, 本研究並依據此三項量化指標進行前測、後測、追蹤測之統計分析, 以回答「藥物治療合併心理教育介入方案,是否優於單獨藥物治療?」
量化研究結果顯示:心理教育介入方案相較於單獨藥物治療,改 善程度有明顯差異。質性研究部份,則藉由深度訪談資料分析,探究 身體症狀疾患之疾病概念、自我概念、家庭因素、治療概念、社會因 素等綜合影響與漫長過程中,發生的改變歷程與其與身體症狀之交互 作用。並萃取出心理教育介入方案影響治療成效之因子,以應用於日 後治療實務。
The aim of this study is to elaborate the effective treatment strategy for somatic symptoms disorder (SSD). This study recruit ten adolescent and young adults who have been diagnosed as SSD. We divided these ten cases into two groups by their choice. The first group received medical treatment only and the second group received medical treatment and psychoeducational intervention. We evaluate the severity of SSD by three clinical scales: Clinical Global Impression–Severity Scale (CGI-S), Social and Occupational Functioning Assessment Scale (SOFAS), and Psychosomatic Symptoms Questionnaire (PSQ-39). According to these statistic results of this study, the psychoeducational intervention combined with psychopharmacological therapy is more effective than the psychopharmacological therapy only for SSD. Therefore, we review the qualitative data in depth from interview sessions. Furthermore, we extract the improving and exacerbating factors from cases who received
psychoeducational intervention.
