目的:
從2019年新型冠狀病毒COVID-19爆發以來,鑒於新冠病毒的傳播力、無症狀感染以及變種病毒隨著時間增加,接種新冠疫苗是潛在遏止大流行的重要措施之一,所以施打疫苗仍是臺灣防疫政策勢在必行的趨勢。因臺灣防疫政策中,雖會與中央流行疫情指揮中心一致,但各縣市疫情狀況可能不同,故地方的防疫規定也會有其個別性,因筆者於高雄區的醫學中心執行臨床護理業務,所以本研究將會以高雄區公立醫院急診部單位醫護人員(醫師、護理師、專科護理師)作為研究對象,探討COVID-19及其疫苗的知識、態度及預防COVID-19與接種COVID-19疫苗行為之間的關係。
方法:
本研究以橫斷性研究,採用問卷調查法,收案時間由2022年5月13日至2022年7月22日止,調查高雄地區公立醫院的急診單位醫護人員的基本人口學變項、健康行為、過去接種經驗以及對於COVID-19疫情的知識、態度及預防COVID-19與接種其疫苗行為,發放問卷計200份,收回問卷有179份,剔除無效問卷13份,有效問卷共計166份,以SAS統計軟體進行分析。
結果:
結果發現不同年齡層及不同急診工作年資在知識總分上有顯著差異;職務別與性別的不同在罹患性、嚴重性與障礙性方面的態度會有顯著差異;不同教育程度在態度總分、利益性與障礙性方面的態度分數有達到統計上的顯著差異;急診工作年資的不同對態度總分有顯著的差異;有無飲酒習慣與定期健檢或定期接種流感疫苗習慣對於行為總分有明顯的差異;此外,固定接種流感習慣也對利益性方面的態度分數有顯著的差異。而以複迴歸模型探討急診醫護對COVID-19的相關知識與各面向的態度分數的關聯性可以發現醫護人員對COVID-19的相關知識分數每增加1分,其對COVID-19的態度分數會增加3.08分,其中利益性態度會增加1.72分、障礙性態度會增加1.03分。探討醫護人員對COVID-19的態度與預防COVID-19及接種疫苗意願之關聯性,從結果中可以發現急診醫護人員對COVID-19的態度總分、利益性態度與障礙性態度每增加1分,預防COVID-19及接種疫苗的意願分數會分別增加0.10、0.18與0.21分。
結論:
從以上結果可以得知,急診醫護人員對於COVID-19及其疫苗的知識越正確,對於COVID-19及其疫苗的態度就會越正向,而態度越高分,面對於預防COVID-19及接種COVID-19疫苗的行為意願則越高。故可以建議醫療院所或是政府衛生單位加強衛教及宣導COVID-19及其疫苗的知識,從針對醫護人員不願意施打COVID-19疫苗的原因去做原因分析,並排除其疑慮,未來可以不只針對醫護人員作為宣導,甚至可以將其結果套用在一般民眾上,或許從醫護人員到一般民眾對於預防COVID-19及其疫苗的態度與行為越趨正向,接種比率增加,導致國內疫情可以逐漸趨緩。
Purpose:
Since the outbreak of the new coronavirus COVID-19 in 2019, in view of the spread of the new coronavirus, asymptomatic infection, and the increase in mutant viruses over time, vaccination against the new crown is one of the important measures to potentially curb the pandemic. Since the author performs clinical nursing services in a medical center in Kaohsiung District, this study will use the medical staff (physicians, nurses, nurse practitioners) in the emergency departments of public hospitals in Kaohsiung District as the research objects to explore the knowledge of COVID-19 and its vaccines, attitudes, and the relationship between COVID-19 prevention and COVID-19 vaccination behavior.
Method:
This study is a cross-sectional study, using the questionnaire survey method. The time of acceptance is from May 13, 2022 to July 22, 2022. The variables include basic demographic variables, health behavior, past vaccination experience, knowledge, attitudes towards COVID-19 epidemic, prevention of COVID-19 and vaccination behavior. A total of 200 questionnaires were distributed, 179 questionnaires were returned, 13 invalid questionnaires were excluded, and a total of 166 valid questionnaires were collected. Analysis were performed with SAS statistical software
Result:
The results show that there are significant differences in the attitudes of predisposition, severity, and hindrance depending on the position and gender; there are statistically significant differences in the average attitude scores, benefits, and obstacles of different education levels ;There are significant differences in knowledge scores and attitude total scores with different years of emergency work. Mean knowledge scores vary significantly across age groups. Whether there is a habit of drinking alcohol and the habit of regular health checkup or regular flu vaccination has a significant difference in the total score of behavior. In addition, regular flu vaccination habits also had significant differences in attitude scores for benefits. However, using a multiple regression model to explore the association between emergency medical staff''s knowledge about COVID-19 and their attitude scores in various aspects, it can be found that for every 1-point increase in medical staff''s knowledge about COVID-19, their attitude towards COVID-19 scores will be increased by 3.08 points, of which the benefit attitude will increase by 1.72 points, and the obstacle attitude will increase by 1.03 points. To study also explored the association between the attitudes of medical staff towards COVID-19 and the willingness to prevent COVID-19 and vaccination. From the results, it can be found that for every 1 point increase in emergency medical staff''s attitude scores, benefit attitudes, and barrier attitudes towards COVID-19, The willingness to prevent COVID-19 and get vaccinated will increase by 0.10, 0.18 and 0.21 points, respectively.
Conclusion:
From the above results, it can be known that the more accurate the emergency medical staff''s knowledge about COVID-19 and its vaccine, the more positive their attitude towards COVID-19 and its vaccine, and the higher the attitude score, the more effective they are in the prevention of COVID-19 and its vaccine. The behavioral willingness to get vaccinated against COVID-19 is higher. Therefore, it can be suggested that medical institutions or government health units strengthen health education and publicize the knowledge of COVID-19 and its vaccines, analyze the reasons why medical staff are unwilling to administer COVID-19 vaccines, and eliminate their doubts. In the future, it can not only be used as advocacy for medical staff, but can even be applied to the general public. When the attitude and behavior of the medical staff and the general public towards the prevention of COVID-19 and its vaccine will become more positive, leading to the gradual spread of the domestic epidemic slow down.
