近年來有許多創傷相關文獻或書籍探討童年逆境經驗與成年後之行為、健康和學習間的關聯,像是在兒童及青少年發展過程中遭遇到依附關係中斷、長期人際創傷,生理或情感的虐待,及經歷到剝奪引起的症狀,嚴重影響他們往後的成長,造成神經系統損傷,反映在學習、認知以及情緒行為上,或是發生身心疾病。許多研究也指出,音樂可有效治療或改善情緒失調,並透過音樂治療恢復與人的正向連結,改善生心理、社會、健康等問題。因此本研究欲運用發展性創傷的概念,融入Van der Kolk(2015)提及之創傷工作中的四項具體步驟,並遵循美國物質成癮和心理健康署提出的創傷介入重要原則,設計音樂治療活動,作為具童年逆境史之青少年的介入。
研究目的:探討音樂介入團體對具創傷史青少年的情緒症狀、同儕關係及孤獨感的影響。
研究方法:本研究採單組前後測,以立意取樣邀請8名具創傷史的青少年,進行8週音樂介入團體,並以團體施測方式進行長處和困難量表(Strengths and Difficulties Questionnaire, SDQ)、孤獨量表(UCLA Loneliness Scale Version3, UCLA _LS_V3)前後測量及個別訪談來進行介入的成效分析。
研究結果:音樂介入後對青少年的情緒症狀、同儕關係及孤獨感的影響雖未達到統計學上之顯著效果,但情緒症狀及孤獨感在介入後仍有些微進步。而從每次團體前的心情溫度計量表來看,八次課程有六次在結束課程後,情緒症狀明顯下降。此外,在質性資料上顯示,受試者對音樂介入的歷程產生進步與改變,並給予正向的回饋與反應。

In recent years, many trauma-related literature or books have explored the relationship between Adverse Childhood Experiences (ACE) and behavior, health, and learning in adult. Van der Kolk (2005) proposed the term Developmental Trauma Disorder(DTD), to explain that children and adolescents encounter disruptions in attachment relationships, long-term interpersonal trauma, physical or emotional abuse, and experience symptoms of deprivation. it will affect their future growth seriously, causing damage to the nervous system, which is reflected in learning, cognition, emotional behavior, or the occurrence of physical and mental diseases. Many studies have pointed out that music can treat or improve emotional disorders effectively, recover positive connections with people through music therapy, and improve physical, psychological, social, health and other problems (Aalbers et al., 2019; Hou et al., 2017 ; Kolacz et al., 2019), therefore this study intends to use the concept of developmental trauma, integrate it into the four specific steps in trauma work mentioned by Van der Kolk (2015), and follow the important principles of trauma intervention from the Substance Abuse and Mental Health Services Administration(SAMHSA) to design activities of music therapy as an intervention for adolescents with childhood adversities.
Purpose:
To explore the effects of music intervention group on adolescents with maltreatment history on emotional symptoms, peer relationships and loneliness.
Methods:
This study adopts one-group pretest-posttest design, and uses purposive sampling to invite 8 adolescents with maltreatment history to participate in an 8-week music intervention group. The Strengths and Difficulties Questionnaire (SDQ) and UCLA Loneliness Scale Version3 (UCLA _LS_V3) were administered in a group manner before and after the intervention. Also, individual interviews were applied to analyze the effectiveness of the intervention.
