本研究乃是針對變性慾症患者接受變性手術後的身心與社會適應狀況
所作的追蹤研究, 間接用來探討耗時耗資的變性手術對變性慾症治療的價
值, 並且作為施行與照顧變性手術患者的參考 本手術以立意取樣, 共
六位變性慾症患者接受追蹤, 其中男變女一位與女變男五位。年齡介於27
歲到37歲之間; 教育程度集中在高中職、大專與研究所。距變性手術完成
的時間則在二年至五年之間。本研究採用回溯性追蹤個案研究 法。研究
工具, 以深度訪談二至五小時為主, 而以包含術前術後發展史的半結構化
問卷為輔。 本研究發現:(1) 追蹤狀況:至八十四年底, 接受某醫學中
心全程變性手術且病歷完整者共112人.但追蹤八十六年五月,包括電話錯
誤或搬家等流失之個案高達57人,佔50.9%; 包括個案或家屬拒絕、與個案
遠行無法聯絡以致聯絡失敗者共20人,佔17.8%; 而同意或已經接受訪談者
共23人, 只佔20.6%.(2) 以質性資料來分析,患者術後適應的七大主題分
別是:打開心內的窗、掙脫舊枷鎖、重生、面對新身體、維繫變性朋友的
矛盾情結、敏感的親密關係與暴光的隱憂.(3) 生理適應方面、手術後之
併發、如漏尿與狹窄,是手術後最主要的壓力源.大部分對性功能期待能更
加強.(4) 心理適應方面,新的性別角色的扮演不成問題.但在親密關係部
份、有部份個案表示因為受變性身分的影響,會使目信心減羽,交往過程中
也較敏感.手術最大的意義在於對心理狀態,特別是情緒方面,有提昇的果.
(5) 社會適應方面,手術後因身分證問題的就業困擾減少,家人關係在術後
改變幅度不大; 朋友關係以往拓展, 也不會侷限在同是變性朋友中. 但親
密關係部份則個案間差異很大. (6) 整體評價部份, 所有接受訪談之個案
均表示不後悔接受變性手術. 雖然本追蹤研究受到個案數的限制, 但
追蹤結困顯示, 手術對個案之心理狀態、社會關係與工作有正面的價值.
而手術併發症的後續處理、以及再造器官外觀與性功能提昇, 是變性慾症
患者術後深切的希望. 但是手術後仍有新的適應問題,特別是面對社會對
變性慾症的歧視,個案在親密關係開展上不如意者居多。 本研究同時也顯
示, 社會對性別的刻板印象同樣的深刻影響了變性慾症患者認者與行為模
式。

Objective:Transsexualism is the most servere form of gender
dysphoria.The patients suffer from disharmony of body and soul,
and result in interpersonal, and occupationaldysfunction.
Psychotherapy and hormomr therapy are often less satisfied than
sexreassignment surgery (SRS). In Taiwan, the first SRS was
performed on 1988, and thereare up to 150 persons who received
SRS till now. What is the sequel and how transsexualsadapt after
SRS is the main focus of this study.Methods: Retrospective study
was adopted. In-depth interview (from 2 to 5 hours) and semi-
structured questionnaire were administrated. Thhe subjects were
those came from the 112 patients who had completed the SRS in a
medical center and were willing to be interviewed. Five female-
to-male transexuals and one male-to-female transexual
wereincluded. They were aged from 27 to 37 years old and one
female-male transsexual had married. They all had jobs. The time
of this follow-up were ranged from 2 years to 5 years after SRS.
Result: (1) The loss follow-up rate was 50.9%, and the refusal
rate was 17.8%. Only 23(17.8%) operated transsexuals were
willing to be interviewed. (2) The themes of adptation,by
qualitative analysis, were: opening the windows of heart, trying
to throw off the oldfetters, re-bom, facing the new body, the
ambivalence to maintain the freendship of transsexual friends,
the fragile and susceptible intimacy, and the fear of
disclosure.(3) Postoperatively, the complications, such as
fistula and urethral stenosis, were most stressful events ,and
sexual function was desired. (4) The new gender role was well
adapteddue to preoperative real-life test. However, low self-
confidence and sensitive attitudewere confronted and made
hesitate if a new intimate relationship was developing. (5) The
occupational barrier were lessened due to new gender role and
new legal certification. The intra-family relationship remained
well and the interpersonal relationship broadened. The intimacy,
which they also desired, revealed great difference between
subjects. (6) Nosubject was regretful to be operated.Discussion:
The SRS was valuable for the postoperative mental status, social
relationship,and occupation. Preventing complications, and
promoting cosmic appearance and sexualeven to productive
function were desired. However, new problems would be
encountered.Especially under the discrimination of
transsexualism, our subjects were hard to ownintimacy. In
addition, this study confirmed that the gender stereotype of
this societywas also influencing the cognitive and behavioral
pattern of transsexuals.
