本論文分為兩大部分：第一部份為門診藥品部分負擔對老年民眾醫療影響之實證研究，第二部份嘗試以成本會計差異分析模型解釋藥費成長之原因，分別如下：
第一部分－
本部分之主要目的在探討門診藥品部分負擔實施後，對老年民眾醫療利用與醫療支出之影響。本研究的資料取自中央健保局，採用實施前後加控制組之設計，樣本期間以民國88年8月1日實施門診藥品部分負擔為區隔，取得87年8月至88年3月以及88年8月至89年3月共16個月資料，而研究母體為隸屬於中央健康保險局之一分局之醫事機構，以抽樣方式選取2家醫學中心、4家區域醫院與15家地區醫院，並以描述性統計、卡方檢定及ｔ檢定之統計方法進行分析。本部分之主要實證發現為：藥品部分負擔實施後，老年人樣本期間總藥費仍呈上漲情形，惟就整體而言，部分負擔組成長率較免部分負擔組顯著為低；平均處方箋藥費是造成總藥費增加的主因，有高價處方增多之情形；就慢性病組別而言，藥品部分負擔對慢性病患抑制效果較差，在非慢性病組別中，部分負擔組的藥品利用成長情形明顯受到抑制。
　　由於本研究期間僅有實施前後各八個月份，未能完全窺見門診藥品部分負擔制度對老年人醫療利用之影響，故藥品部分負擔制度對老年民眾醫療利用及健康狀況之影響，仍難定論。
第二部分－
本部分嘗試以成本會計之差異分析模型之觀念引進藥費分析中，將各個影響藥費變化之因素全盤加以考量。本研究之研究期間包括藥品部分負擔制度實施後八個月與前一年相同月份八個月共16個月資料，比較制度實施前後藥費變化之因素。並將示範此一藥費差異分析模型亦可套用至各分析者欲分析之樣本中，均可得出有用之結果。
差異分析結果提供了藥費控制另一思考方向，可針對主要因素進行控管，此一分析工具擬提供給衛生主管機構在政策制定時作為一項可用之分析工具。

The thesis consists two parts：Part one is entitled “The Impact of Outpatient Drug Cost-Sharing On Medical Utilization and Medical Expenditure for the Elderly in Taiwan”, and part two is entitled “Drug Costs Analysis Model”. The abstract of each is as follows.
Part One
The purpose of this part is to investigate the effect of the outpatient drug cost-sharing program on medical utilization and medical expenditure of the elderly persons in Taiwan. As the outpatient drug cost-sharing policy was enacted on August 1,1999,the sample period chosen is August, 1999-March, 2000 and the corresponding period in 1998-1999 to control for the monthly effect. The study adopts a pretest-posttest control group design by sampling method. Twenty-one hospitals randomly selected from Taipei branch of the Bureau of the National Health Insurance are included in the sample, consisting of 2 medical centers, 4 regional hospitals, and 15 district hospitals. The data was analyzed using Chi-square tests and t-tests. Major findings of this part are summarized as follows：After implementing the policy, total drug costs still increase. But as a whole, the increase rate for the cost-sharing group is significantly lower than the non cost-sharing group. Average drug cost per prescription has the main effect on the increase of total drug cost. The effects of drug cost-sharing on the chronic diseases group is significantly weaker than the effect of the non-chronic disease group.
Part Two
This part tries to introduce the variance analysis model to explain overall effects that affect drug costs. As the outpatient drug cost-sharing policy was enacted on August 1,1999,the sample period chosen is August, 1999-March, 2000 and the corresponding period in 1998-1999 to control for monthly effect. This part illustrates that cost variance analysis model commonly use in managerial accounting is useful in analyzing drug policy. Major findings of this part are summarized as follows：Concerning changes in total drug costs for the whole sample, average drug price per prescription is the main factor which explains the increase in drug costs for 65.12%; and average drug quantity per prescription, average drug item per prescription, average quantity per day per drug item all result in a decrease in drug costs. The results of the cost-sharing group and the whole sample are similar in terms of cost variances. The main factor which drives an increase in drug costs for the non cost-sharing is the average drug day per drug item, explaining drug costs increase up to 64.79%.
