健康飲食是促進健康首要條件之一,飲食攝取是個體的生理需求與經濟環境、社會網絡、文化信仰和生活型態互相影響的結果。社會及經濟條件,會造成飲食品質的差異,可能進一步影響健康的平等。透過了解影響飲食的社會決定因素,可幫助我們訂定健康促進措施時,針對目標群體,消弭飲食不平等,縮短後續的健康差距。本研究利用四次國民營養健康狀況變遷調查資料,以ODI-R和YHEI-TWR為整體飲食品質指標,比較歷次調查間之變化,並分析影響國人飲食品質的社會人口學、行為、健康特性。資料以One-way ANOVA、多元線性迴歸進行分析,並利用SUDDAN統計軟體加權並校正抽樣效應。結果顯示,國中生與19-64歲成人飲食品質變佳,但高中職生飲食品質降低;就青少年而言,父母同居、社經地位愈高、未抽菸、看電視時間愈少者,飲食品質愈佳。19歲以上成人部分,女性、年齡、社經地位及體能活動愈高、服用補充劑者,飲食品質愈佳;原住民、抽菸、嚼食檳榔者飲食品質較差。雖然飲食不平等差距仍然存在,然而,氏族與社經地位對國人所造成的飲食品質差距與不平等,正逐漸弭合、消除。
Healthy diet is an important focus of health promotion. Diet is a complex interplay between individuals’ physiological requirement, economic circumstances, social networks, cultural beliefs and lifestyle behaviours. However, social econamic status difference may lead to social gradient in diet and health eventually. Therefore, in order to reducing dietary inequalities and closing health gap in future, research that clarifies the determinants of dietary behaviour should help us target health promotion initiatives more effectively. The aim of this study was to investigate changes in diet quality among Taiwanese, and to explore its association with socio-semographic, behavioral, and health characteristics. The Overall Dietary Index Revised (ODI-R) scores and Youth Healthy Eating Index TAIWAN Revised (YHEI-TWR) scores were estimated using 24 hour dietary recall data and FFQ provided by participants in the four Nutrition and Health Survey in Taiwan (NAHSITs) 1993-1996 to 2010-2011. The results showed that junior high school student and adult’s diet qualities have increased, while senior high school student’s has decreased. Among adolescent, diet quality was higher in parents cohabiting, high parents’ social econamic status, non-smoker, less TV-watching time. Among adult (19+ years), diet quality was higher in women, older age, high social econamic status, high physical activity, dietary supplement taker, while lower in aboriginal, smoker, betel nut chewer. Although social-demographic disparities in diet quality persist, ethnicity and social econamic status gaps in the score narrowed over this period.
