中文摘要
根管充填劑依材料組成的不同 , 可分為(1)氧化鋅丁香油酚基底 (ZOE based) , (2)氫氧化鈣基底 (Ca(OH)2-based) , (3)樹脂基底 (Resin-based)三種充填劑。其本身之生物相容性可說是根管治療成功的關鍵之一 , 因為毒性物質可能會延誤根尖周圍組織之癒合 , 或甚而引發發炎反應。回顧文獻也發現諸多探討根管充填劑毒性之研究 , 但其結果並不儘相同 , 為能更加掌握根管治療療程的進行 , 實在有必要對根管充填劑做進一步的瞭解 ; 另外 , 近年來的牙科材料雖然經過許多改良 ,且其細胞毒性也因而減少 , 但是因為所產生的抗菌物質 , 而導致組織細胞發生細胞凋零(Apoptosis) 的現象 , 也引起我們對於根管充填劑是否會引起細胞凋零(Apoptosis) 的問題產生興趣。因此本研究之目的即藉著細胞培養的方法 , 以MTT assay來評估各類型之根管充填劑(ZOE based; Calcium hydroxide based; and Resin based sealers) ,了解其對細胞之生物相容性 , 以做為臨床治療上選擇的參考依據。並且利用DNA 碎片(DNA fragment)在電泳膠上形成的梯形條紋(ladder)來觀察細胞凋零(Apoptosis) 的現象。結果發現AH26和AH plus具有較強的細胞毒性 ; 而 Canals-N則具有較佳之生物相容性。但是所有充填劑的毒性 , 都隨著劑量和時間的增加而增加(p<0.05)。這些根管充填劑毒性所導致的細胞死亡可能可經由細胞凋零(Apoptosis)產生的。
所以在臨床根管治療時 , 無論使用何種根管充填劑 , 都要注意根尖狹窄 ( apical constriction ) 的維持 , 儘量減少根管充填劑和根尖周圍組織的接觸 , 以降低細胞毒性反應的影響。
