目的: 台灣缺乏評量兩歲半以前兒童詞彙發展的工具,因此本研究將MacArthur 溝通發展量表(MacArthur Communicative Developmental Inventory)的CDI--WS版本加以修訂,成為一份具有信度與效度且符合測量台灣南部兒童詞彙的MacArthur溝通發展量表中文版,以此量表為工具比較早產兒與足月產兒在詞彙發展上的差異。
受試者與方法: 研究對象為2000年12月至2001年8月於嘉義基督教醫院或嘉義聖馬爾定醫院出生或住院的早產兒,早產兒的選取標準為懷孕週數小於34週,調整後年齡為18至24個月大。依出生體重將早產兒分為低出生體重早產兒(LBW)14人(生理年齡:21.21±1.37個月;平均懷孕週數:32.57±1.09週;平均出生體重:1800.93±261.13公克),極低出生體重早產兒(VLBW)6名(生理年齡:21.5±1.38個月;平均懷孕週數:28.33±2.88週;平均出生體重:1099.5±201.11公克)及足月產兒童21名(生理年齡:21.14±1.96個月;平均懷孕週數:38.42±1.17週;平均出生體重:3121.67±367.58公克)。利用貝萊氏嬰幼兒發展測驗(Bayley Scale)來瞭解早產兒的動作發展指標(PDI)與心智發展指標(MDI),與利用MacArthur溝通發展量表中文版比較三組兒童詞彙發展的差異,並以MDI當共變項來瞭解早產兒的詞彙發展狀況。
結果: 在貝萊氏嬰幼兒發展測驗的分析比較結果,VLBW在PDI、MDI、通過的認知題數和通過的語言題數皆顯著落後於足月產兒童,而LBW僅在MDI與通過的認知題數上顯著落後於足月產兒童,VLBW與LBW在貝萊氏嬰幼兒發展測驗上的表現沒有顯著性差異。在MacArthur溝通發展量表中文版的分析比較結果,不論是總詞彙量、將詞彙分類為一般性名詞、述詞和相關的類別詞彙和21個類別詞彙量的比較,三組兒童皆沒有顯著性差異。
結論: 本研究發現VLBW在貝萊氏嬰幼兒發展測驗上的所有測量皆顯著低於足月產兒童,而LBW僅在MDI與通過的認知題數上顯著落後於足月產兒童。雖在MacArthur溝通發展量表中文版的比較,三組兒童統計上的差異,但足月產兒童的總詞彙量多於所有的早產兒童,LBW早產兒的總詞彙量又多於VLBW早產兒,且由早產兒童與足月產兒童在MacArthur溝通發展量表中文版的各類別表現,可知雖早產兒童的詞彙量低於足月產兒童,但他們詞彙增長的趨勢與足月產兒童相似。
