長期呼吸器依賴患者整合照護計畫實施至今，慢性呼吸照護病房病患人數成長率遠高於居家照護的人數，儘管居家照護較節省醫療成本也較人性化，但仍無法提高家屬的意願。因此本研究擬探討慢性呼吸照護病患轉居家照護之困難原因。

研究設計為量化與質性研究之混合設計，量化研究採立意取樣以結構式問卷收集資料，對象為雲嘉地區慢性呼吸照護病房患者家屬，得有效問卷129份。所得資料以SPSS 20.0 版進行描述性統計分析、獨立樣本 t 檢定、單因子變異數分析及皮爾森相關係數進行統計分析。質性研究採個別半結構式訪談法，選取五位雲林某區域教學醫院慢性呼吸照護病房患者之家屬進行訪談。量化與質性研究分別進行資料分析後，將結果進行對照。

研究結果發現，慢性呼吸照護病房患者之家屬拒絕病患轉居家照護最主要的原因為「害怕病人可能隨時發生緊急狀況」，質性研究結果也和量化研究結果相符。另一方面，當家屬年齡較大、教育程度較低、經濟狀況差、健康狀態不佳、沒有人可以分擔照護責任、病患生病前為家中主要經濟來源、入住加護病房次數較多及病患為植物人或昏迷狀態等家屬各層面困境顯著較高。並且家屬不希望病患返家接受居家照護的原因在生理、心理、靈性及社會層面間皆呈現顯著正相關。

針對研究結果提出了給長期呼吸器依賴患者家屬的手冊、成立呼吸器依賴患者家屬支持團體、醫療單位的評估與介入、完整的出院準備服務、發展居家呼吸照護遠距醫療、長期照護保險的制定及安寧善終等建議，期能透過改善家屬各層面的困境來提升家屬讓病患轉居家呼吸照護之意願。

Due to the implement of Integrated Delivery System & Prospective Payment System (IDS project), growth rate of respiratory care ward (RCW) patients were higher than home care patients. Although medical cost of home care was fewer than RCW, in addition, home care was more humanized, however, it cannot enhance the will of family in using home care. The purpose of this study was to investigate the difficulties of the patients in RCW transfering to home care.

This is a quantitative and qualitative mixed design. Quantitative study used purposive sampling by using a structured questionnaire to collect information. The sampling subjects are from RCW in Yunlin and Chiayi. There are 129 valid samples. The collected data is analyzed with descriptive statistics, t-test, one-way ANOVA and Pearson’s correlation by using SPSS version 20.0. Qualitative study used semi-structured personal interview. Selected five families as interviewer from a RCW in Yunlin regional teaching hospital. After quantitative and qualitative data were analyzed, the results were compared.

The results found that the main reason of RCW family reject ventilator-dependent patients transfering to home care is the family were afraid of emergency at anytime. These results were identical in qualitative and quantitative research.Otherwise, the family encounter great difficulties when they getting older, having lower education level, worse economic state worse health, lacking of assistant in attending patients, patients were economic pillar before they were sick, transferring to ICU more frequently, patients were vegetable or coma. The reasons that the family refused patient transfering to home care in physical, mental, spiritual, and social levels had significant positive correlation.

The results of this study suggest that a printed information brochure and support groups for family of ventilator-dependent patients, evaluation and intervention from medical units, complete discharge planning service, telemedicine, long-term care insurance, and hospice palliative care should be provided. This study expects that the suggestions may enhance family’s will to transfer patients to home care through assisting them in overcoming difficulties.
